Digital healthcare offers a promising solution for bridging the equity gap and leveraging technology to deliver healthcare services remotely to vulnerable populations. Focus group discussions (FGDs) were conducted to enrich our understanding of how variables previously identified in the literature fit the research context. Eleven research hypotheses that were developed were tested using quantitative methods. Results highlight that customer operant, clinical, personal network, and digital resources enhance participation behavior in digital healthcare. This study provides insights into how participation behaviors improve subjective well-being and effectively strengthen loyalty through the mediating role of subjective well-being.

Health literacy, digital health literacy and the implementation of digital health technologies in cancer care: the need for a strategic approach.

Digital health technologies can potentially reduce health disparities in cancer care. However, the benefits of digital health technology depend partly on users' digital health literacy, that is, "capabilities and resources required for individuals to use and benefit from digital health resources," which combines health and digital literacy. We examined issues for digital health technology implementation in cancer care regarding digital health literacy, via stakeholder consultation. Consumers, health care professionals, researchers, developers, nongovernment and government/policy stakeholders (N=51) participated in focus groups/interviews discussing barriers, enablers, needs and opportunities for digital health implementation in cancer care. Researchers applied framework analysis to identify themes of digital health literacy in the context of disparity and inclusion. Limited digital and traditional health literacy were identified as barriers to digital technology engagement, with a range of difficulties identified for older, younger and socio-economically or geographically disadvantaged groups. Digital health technology was a potential enabler of health care access and literacy, affording opportunities to increase reach and engagement. Education combined with targeted design and implementation were identified means of addressing health and digital literacy to effectively implement digital health in cancer care. Implementing digital health in cancer care must address the variability of digital health literacy in recipients, including groups living with disadvantage and older and younger people, in order to be effective. SO WHAT?: If cancer outcome disparity is to be reduced via digital health technologies, they must be implemented strategically to address digital health literacy needs. Health policy should reflect this approach.

The Relationship between Healthcare Digital Marketing Exposure and Health Improvement Practices among Students at a Medical School in Davao: A Mixed Methods Study

Aims: This study explores the relationship between healthcare digital marketing exposure and health improvement practices among Grade 12 students in the Accountancy, Business, and Management (ABM) strand. It also aims to clarify how digital marketing influences their health-related behaviors, providing insights relevant to both educational and healthcare sectors. Study Design: This was a mixed-methods study utilizing an explanatory sequential design. Place and Duration of Study: The research was conducted at a medical school in Davao City from August to December 2024. Methodology: The study involved 66 Grade 12 ABM students who participated in the quantitative phase through a researcher-made survey, selected using total sampling. For the qualitative phase, 15 students were chosen through purposive sampling and participated in semi-structured Focus Group Discussions (FGDs). Quantitative data were analyzed using statistical tools such as mean and Pearson r test, while qualitative data were examined using thematic analysis. Triangulation was applied to compare and validate the findings from both phases. Results: A significant positive relationship was found between exposure to healthcare digital marketing and the adoption of health improvement practices (p < 0.05). Students reported moderate exposure to healthcare digital marketing (mean = 3.34) and high engagement in health improvement practices (mean = 3.46). Thematic analysis from the FGDs supported these statistical results, with participants stating that exposure to digital healthcare content through platforms such as social media, healthcare websites and blogs, and email marketing influenced their decisions related to diet, exercise, and health product consumption. Conclusion: The study highlights the relevance of digital marketing in shaping youth health behaviors. Findings suggest that targeted digital health campaigns can effectively influence students' wellness practices. Practically, the study underscores the need for integrating digital health literacy programs in the curriculum to empower students to critically assess online health information and adopt healthier lifestyles.

Developing Health Management Competency for Digital Health Transformation: Protocol for a Qualitative Study

BackgroundGlobally, the health care system is experiencing a period of rapid and radical change. In response, innovative service models have been adopted for the delivery of high-quality care that require a health workforce with skills to support transformation and new ways of working.ObjectiveThe aim of this research protocol is to describe research that will contribute to developing the capability of health service managers in the digital health era and enabling digital transformation within the Australian health care environment. It also explains the process of preparing and finalizing the research design and methodologies by seeking answers to the following three research questions: (1) To what extent can the existing health service management and digital health competency frameworks guide the development of competence for health service managers in understanding and managing in the digital health space? (2) What are the competencies that are necessary for health service managers to acquire in order to effectively work with and manage in the digital health context? (3) What are the key factors that enable and inhibit health service managers to develop and demonstrate digital health competence in the workplace?MethodsThe study has adopted a qualitative approach, guided by the empirically validated management competency identification process, using four steps: (1) health management and digital health competency mapping, (2) scoping review of literature and policy analysis, (3) focus group discussions with health service managers, and (4) semistructured interviews with digital health leaders. The first 2 steps were to confirm the need for updating the current health service management curriculum to address changing competency requirements of health service managers in the digital health context.ResultsTwo initial steps have been completed confirming the significance of the study and study design. Step 1, competency mapping, found that nearly half of the digital competencies were only partially or not addressed at all by the health management competency framework. The scoping review articulated the competencies health service managers need to effectively demonstrate digital health competence in the workplace. The findings effectively support the importance of the current research and also the appropriateness of the proposed steps 3 and 4 in answering the research questions and achieving the research aim.ConclusionsThis study will provide insights into the health service management workforce performance and development needs for digital health and inform credentialing and professional development requirements. This will guide health service managers in leading and managing the adoption and implementation of digital health as a contemporary tool for health care delivery. The study will develop an in-depth understanding of Australian health service managers’ experiences and views. Understanding Components of Therapeutic Alliance and Well-Being from Use of a Global Digital Mental Health Benefit During the COVID-19 Pandemic: Longitudinal Observational Study

Digital mental health services leverage technology to increase access to care, yet less is known about the quality of therapeutic relationships in a virtual setting. This study examined components of therapeutic alliance (a mechanism underlying successful treatment) and its association with beneficial treatment outcomes in a real-world, virtual setting. The objective is to examine (1) participant ratings of components of therapeutic alliance with providers in a virtual setting, (2) changes in subjective well-being and depressive symptoms among participants who began care with elevated depressive symptoms, and (3) the association between components of alliance and changes in participants’ well-being. Adults (N = 3,087, M age = 36 ± 9 years, 54% female) across the world with access to digital mental health benefits who engaged in videoconference sessions with a licensed therapist (18%, 555/3,087), certified coach (65%, 2,003/3,087), or both (17%, 529/3,087) between Sept. 29, 2020 and Oct. 12, 21. Participants completed 2 adapted items from the Working Alliance Inventory (goals and bonds subscales) after each session, and ratings were averaged across visits (Cronbach’s ɑ = .72). Participants’ World Health Organization-Five (WHO-5) Well-Being Index scores at the start and end of the study period were used to measure changes in subjective well-being. Descriptive and inferential statistics were conducted to examine average alliance ratings across demographics and utilization types and the association between alliance and well-being. The median adapted therapeutic alliance score was 4.8 (range: 1–5) and did not differ by age, country, or baseline well-being (Ps > .07). Females reported higher components of alliance than males (4.88 vs. 4.67, P = .01). Participants utilizing telecoaching reported higher components of alliance than those utilizing teletherapy or both telecoaching and teletherapy (4.83 v. 4.75, P = .004), though effect sizes were negligible. Among those with elevated baseline depressive symptoms (n = 835), participants reported an average WHO-5 increase of 15.42 points (95% CI 14.19–16.65, P < .001, Cohen d = 1.06) with 58% (485/835) reporting clinical recovery and 57% (481/835) reporting clinical improvement in depressive symptoms. Higher components of therapeutic alliance scores predicted greater well-being at follow-up (b = 2.04, 95% CI 0.09–3.99, P = .04) after controlling for age, sex, baseline WHO-5, and number of days in care (R2 = .06, P < .001). Exploratory analyses indicated this association did not differ by utilization type, baseline well-being, or session utilization (Ps > .34). People with access to one-on-one videoconferencing care via a digital mental health benefit formed a strong bond and sense of alignment on goals with both coaches and therapists. Higher components of alliance scores were associated with improvements in subjective well-being among participants who began care with elevated depressive symptoms, providing evidence that a positive bond and goal alignment with a provider are two of many factors influencing virtual care outcomes. Continued focus on the quality of therapeutic relationships will ensure digital mental health services are patient-tailored as these platforms expand equitable access to evidence-based care.

The Digital Health Landscape in Addiction and Substance Use Research: Will Digital Health Exacerbate or Mitigate Health Inequities in Vulnerable Populations?

Novel and emerging digital health technologies are increasingly used in substance use and addiction-related self-management and treatment research. The promise of digital health is exciting, yet there are important factors regarding population characteristics to consider prior to using novel technologies with vulnerable populations. This paper reports a review of scientific literature published between 2015 and early 2020 on the use of digital health strategies in research focused on substance use and addiction in vulnerable populations. Using 13 search terms, three databases were screened for published literature meeting specific inclusion criteria. Common themes expressed across the 32 resulting publications included user acceptability, product reliability, and privacy and security concerns. Implementation of evidence-based frameworks and guidelines is needed to guide future digital health research in vulnerable populations. Guidance should involve robust evaluations of acceptability, feasibility, and clinically meaningful use of digital health in diverse populations experiencing addiction-related health concerns.

The Information and Communication Technology Maturity Assessment at Primary Health Care Services Across 9 Provinces in Indonesia: Evaluation Study

BackgroundIndonesia has rapidly embraced digital health, particularly during the COVID-19 pandemic, with over 15 million daily health application users. To advance its digital health vision, the government is prioritizing the development of health data and application systems into an integrated health care technology ecosystem. This initiative involves all levels of health care, from primary to tertiary, across all provinces. In particular, it aims to enhance primary health care services (as the main interface with the general population) and contribute to Indonesia’s digital health transformation.ObjectiveThis study assesses the information and communication technology (ICT) maturity in Indonesian health care services to advance digital health initiatives. ICT maturity assessment tools, specifically designed for middle-income countries, were used to evaluate digital health capabilities in 9 provinces across 5 Indonesian islands.MethodsA cross-sectional survey was conducted from February to March 2022, in 9 provinces across Indonesia, representing the country’s diverse conditions on its major islands. Respondents included staff from public health centers (Puskesmas), primary care clinics (Klinik Pratama), and district health offices (Dinas Kesehatan Kabupaten/Kota). The survey used adapted ICT maturity assessment questionnaires, covering human resources, software and system, hardware, and infrastructure. It was administered electronically and involved 121 public health centers, 49 primary care clinics, and 67 IT staff from district health offices. Focus group discussions were held to delve deeper into the assessment results and gain more descriptive insights.ResultsIn this study, 237 participants represented 3 distinct categories: 121 public health centers, 67 district health offices, and 49 primary clinics. These instances were selected from a sample of 9 of the 34 provinces in Indonesia. Collected data from interviews and focus group discussions were transformed into scores on a scale of 1 to 5, with 1 indicating low ICT readiness and 5 indicating high ICT readiness. On average, the breakdown of ICT maturity scores was as follows: 2.71 for human resources’ capability in ICT use and system management, 2.83 for software and information systems, 2.59 for hardware, and 2.84 for infrastructure, resulting in an overall average score of 2.74. According to the ICT maturity level pyramid, the ICT maturity of health care providers in Indonesia fell between the basic and good levels. The need to pursue best practices also emerged strongly. Further analysis of the ICT maturity scores, when examined by province, revealed regional variations.ConclusionsThe maturity of ICT use is influenced by several critical components. Enhancing human resources, ensuring infrastructure, the availability of supportive hardware, and optimizing information systems are imperative to attain ICT maturity in health care services. In the context of ICT maturity assessment, significant score variations were observed across health care levels in the 9 provinces, underscoring the diversity in ICT readiness and the need for regionally customized follow-up actions.

Health Service Managers' Digital Competencies: A Conceptual Framework.

Health service managers play a crucial role in managing and leading in the digital health environment. Development of the health management workforce that is ready to lead and manage digital health transformation requires partnerships across sectors, in not only developing workforce competence but also in developing supportive mechanisms that can translate competencies into practice. A framework presenting a systematic approach in enabling the development of a competent health management workforce in the digital health era has recently been published. The purpose of this paper is to explore and discuss the application of the framework in the Australian context, informed by the findings of a PhD research project that uses an empirically validated four-step approach to confirm the health service management workforce development needs in the digital health context. The PhD project has already confirmed: 1) the paucity of Australian Health Informatics Competency Framework (AHICF) competencies being included in Australian health service management postgraduate program curricula; 2) five key strategies that contribute to developing health management workforce competency and capacity; and 3) seven key factors that enable health management workforce development in the digital health context. Further understanding of the barriers and enablers for health service managers to develop capability and manage in the digital health environment, and the factors that influence digital health policy and practice will be developed, by critically analysing findings from focus group discussions with health managers and semi-structured interviews with digital health leaders, to be completed by May 2023.

Competencies Needed for Behavioral Health Professionals to Integrate Digital Health Technologies into Clinical Care: a Rapid Review

This rapid review examines literature on training and competencies for behavioral health professionals to integrate digital health technologies into clinical practice. While the evidence for digital health is growing, research evidence supports its use in behavioral healthcare. Despite this, behavioral health professionals have been slow to integrate technologies into care for various reasons. The aim of this review is to inform behavioral health professionals on competencies needed to support the adoption of digital health technologies. PubMed and Google Scholar were searched for articles from 2010 through 2020. Search terms associated with digital health technologies, behavioral health professionals, and competencies, skills, training, and best practices yielded 1972 articles. Twenty-three articles met inclusion criteria indicating foundational core competencies and data was extracted and organized based on profession and technology platform in order to detect similarities and differences. The findings were used to generate an interdisciplinary approach for the clinical integration of digital health. Recommendations are presented for foundational digital health competencies applicable across behavioral health disciplines, rather than technology- or discipline-specific training. The universal digital health inter-professional competencies identified include: (a) privacy, security, and patient safety; (b) digital health technical skills; (c) ethical and legal considerations; (d) clinical skills; (e) art of therapy and digital health; and (f) administrative tasks. Research is needed to understand if the development of professional digital health competencies helps to improve patients’ behavioral health.

Digital health interventions for improving access to primary care in India: A scoping review.

Access to quality healthcare remains a challenge in low-and middle-income countries. Vulnerable populations with unmet needs face the greatest challenge in accessing primary care for appropriate and timely healthcare. The use of digital technologies can not only strengthen health systems but also improve access to health care, particularly for the vulnerable. This scoping review aims to assess the various digital health technologies and interventions available for improving access to primary care for the vulnerable in India. This scoping review employed the Joanna Brigg Institute's (JBI) guidelines and Arksey and O'Malley's methodological framework. The literature search was conducted in Medline/PubMed, Embase, Web of Science-Core Collection, Scopus, AgeLine, PsycINFO, CINAHL, ERIC, Cochrane CENTRAL, and Cochrane Effective Practice and Organisation of Care (EPOC) Group Specialised Register databases, using the keywords, such as 'Access', 'Healthcare', 'Assistive technology', 'Digital health' 'Vulnerable', 'India' and 'Healthcare technology'. A two-staged screening of titles and abstracts, followed by full-text was conducted independently by two reviewers, using the Rayyan software. Subsequently, the data was extracted from selected studies using a pre-designed and approved extraction form. The data was then synthesised and analysed narratively. The protocol for this review has been registered with open science forum (OSF) registries (https://osf.io/63pjw/). The search yielded about 3840 records, 3544 records were eligible for screening of titles and abstracts. We included seven studies after a two-round screening and identified seven different technological innovations developed to bridge gaps in access to primary care. The commonly used digital health interventions for improving access to primary care were virtual tele-health systems and mHealth applications in-built within an android smartphone or a tablet. Digital health interventions was either used as a standalone tele-health aid or a collaborative system for community workers, primary care physicians as well as the health service users. The purpose of these innovations was to increase awareness and knowledge to access support for specific aspects of healthcare. Virtual primary health care with the specialist in the hub supporting general physicians at the primary health centres in blocks and districts was another such model used for improving access to primary care. Digital health interventions was also used for mass community screening of disabilities, such as persons with hearing disability. To re-imagine a digitally empowered health systems in India, also inclusive of the vulnerable, it is important to inclusively conceptualise, systematically develop and rigorously evaluate any public health interventions including those that are enabled by digital health interventions to bridge the gaps in access to primary care in India. Such a strategy could address the paucity of evidence in public health interventions and provide sustainable strategies to strengthen health systems in India. Evolution and interaction mechanisms of China’s high-performing national healthcare system

ObjectivesThis study aims to identify the dimensions and evolutionary pathways of China’s high-performing national healthcare system, as well as the interaction mechanisms between the digital and traditional healthcare dimensions.MethodsThis study first constructs a high-performing healthcare evaluation index comprising four dimensions: digital healthcare, healthcare resource allocation, healthcare output, and healthcare effectiveness. It next presents a multilevel structural dynamic factor model to examine the evolutionary pathway of China’s national healthcare system. It then analyses the interaction mechanism of each healthcare dimension based on the impulse response function.ResultsFirst, the upward trend in the overall performance of China’s high-performing national healthcare system demonstrates that it is significantly improving. Second, the overall performance of China’s high-performing national healthcare system has been most impacted by healthcare effectiveness and least impacted by healthcare output. The performance is trending upward for digital healthcare and healthcare resource allocation but downward for healthcare output and effectiveness. Third, increasing healthcare resource allocation and output promotes digital healthcare. The improvement in digital healthcare performance significantly and positively impacts healthcare effectiveness, while having weaker effects on healthcare resource allocation and healthcare output.ConclusionsThe performance of China’s high-performing national healthcare system is improving. However, healthcare resource allocation and health outcomes require further optimisation, and the integration capacity of traditional healthcare with digital healthcare must be strengthened.

Organizational Factors Driving the Realization of Digital Health Transformation Benefits from Health Service Managers: A Qualitative Study.

Healthcare is experiencing a global period of profound transformation, and digital health shows the promise of dispensing innovative and contemporary service models. However, changes are required to improve the capabilities of health managers in driving successful digital transformation. This paper aims to explore the organizational barriers that health service managers have encountered when realizing the benefits of a digitally transforming environment. This is part of a larger research study exploring digital competencies for health service managers, with the findings from focus group discussions providing a baseline from which to address the organizational improvements and changes in system capabilities required to assist in realizing the benefits of digital health transformation. The study is qualitative in nature. It employs focus group discussions to gain an in-depth understanding of the experiences and views of health service managers and identify the benefits and barriers that managers have experienced in introducing digital health in their workplace. Barriers encountered in realizing digital health benefits in the workplace were evidenced across five major themes: (1) change resistance and usage, (2) trust and uniformity, (3) resourcing and procurement, (4) digital literacy, and (5) siloed systems. Findings from this study demonstrate that in driving the realization of digital health transformation benefits, health service managers need organizational and system-wide efforts to support managing in the digital health context. The key identified barriers experienced by health service managers include facing human and technical challenges with system adoption and the governance of data-driven decision-making in the digital context. The importance of digital transformation in healthcare is evident and will increasingly become a necessity for organizational survival and success. This study adds important insights into the organizational barriers that health service managers have encountered when realizing the benefits resulting from digital transformation. Addressing these barriers requires macro-, meso- and micro-level system investments. These benefits are enhanced by enabling factors critical for digital health adoption that have been described in key categories involving health system related: (1) policy and system, (2) organizational structure and processes, and human resource management, and (3) people factors. The importance of ensuring the organizational factors driving the realization of benefits in a digitally transforming environment is also addressed and capitalized upon for health service managers.

Background: Childhood malnutrition is a major public health concern in the tribal areas of Andhra Pradesh, India, particularly in the region of ITDA Rampachodavaram. The integration and effectiveness of digital health interventions in these underserved regions are vital for addressing this endemic issue. However, the unique challenges posed by the remote and resource- limited settings of tribal areas make the implementation of such interventions particularly complex. Moreover, the varying complexity of the digital health ecosystem in these areas adds another layer of concern to deploying digital health solutions. Objectives: 1. To explore digital health interventions related to childhood malnutrition in the Integrated Tribal Development Agency (ITDA) of Rampachodavaram. 2. To understand the dynamics of the local health system and the digital health ecosystem in the implementation of these interventions. 3. To examine the role of street-level bureaucrats in implementing digital interventions and identify the barriers and facilitators affecting this process Methodology: The research was conducted in ITDA Rampachodavaram, involving interviews with health system officials and Focus Group Discussions (FGDs) with frontline health workers. The Health System Dynamics (HSD) framework and Lipsky’s theory on street-level bureaucrats were utilized, and enriched with insights into the digital health ecosystem. Results: The study reveals significant systemic, contextual, and human factors influencing the implementation of digital health interventions. Challenges include the complex role of street-level bureaucrats and systemic barriers like inadequate training, technological limitations, and disparities in digital access and literacy.
