Abstract

目的: 探讨LKB1在胃癌组织中的表达及其与肿瘤血管形成的关系并分析其临床意义。 方法: 收集荆州市中心医院2010年1月至2017年5月期间存档的胃腺癌组织标本及其癌旁组织标本各107例,其中男性81例,女性26例。采用免疫组织化学方法检测胃癌组织和癌旁组织中LKB1的表达,分析LKB1低表达与胃癌肿瘤血管形成的关系。 结果: LKB1在胃癌组织中高表达率为44.9%(48/107),低表达率为55.1%(59/107);在癌旁组织的高表达率和低表达率分别为77.6% (83/107)和22.4%(24/107),胃癌组织中LKB1的表达显著低于癌旁组织(P=0.005)。胃癌组织中LKB1低表达与血管内皮生长因子(VEGF)表达(r=-0.332,P<0.01)和肿瘤间质微血管密度(MVD;r=-0.262,P=0.006)呈负相关。LKB1低表达与胃癌浸润深度、病理分期和淋巴结转移相关(均P<0.05);VEGF表达和MVD与胃癌病理分期、胃癌浸润深度和淋巴结转移相关(均P<0.05)。Kaplan-Meier分析显示无病生存期与LKB1、VEGF、MVD以及胃癌组织分化程度、浸润深度、TNM分期和淋巴结转移相关,差异有统计学意义(均P<0.05);COX风险比例回归模型分析发现LKB1表达可以作为影响患者无病生存期的独立风险因素[HR:0.600(0.366~0.984),P=0.043]。 结论: LKB1在胃癌组织中表达降低并且与VEGF和MVD呈负相关,是预测胃癌复发、转移的重要指标。.
