Abstract
目的 通过应用CDE观察患者右心室面积变化率(RVPCA),以评价肥厚型心肌病(HCM)患者的右心室心肌收缩功能(RVSF).方法 采用CDE检查56例HCM患者(HCM组)及60例健康体检者(对照组)的心尖四腔心观(A4CH)、胸骨旁心底短轴观(PSAX)及左心长轴观(PLAX)三个不同切面RVPCA及其均值.结果 与对照组比较,HCM组PLAX、PSAX、A4CH的RVPCA值均降低,其中PLAX、PSAX的RVPCA值两组比较,差异均有统计学意义(t=17.552、12.473,均P＜0.05).HCM组综合RVPCA值明显低于对照组(P＜0.01).HCM组右室游离壁近房室环处心肌纵向收缩期s波波峰速度(Vs)为(10.5±4.1)cm/s,s波速度时间积分(VTIs)为(1.6±0.7)cm;对照组Vs为(13.6±3.8)cm/s,VTIs为(2.3±0.6)cm;两组Vs、VTIs比较,差异均有统计学意义(t=4.262、5.839,均P＜0.01).结论 HCM患者的右心室心肌收缩功能有一定程度减退,通过CDE对HCM患者RVSF进行评价具有可行性。
