Clinical Characteristics and Surgical Outcomes of Acute Acquired V-Pattern Esotropia: A Comparative Study Highlighting Novel Demographics, Refractive Profiles, and Superior Sensory Recovery

ABSTRACT Purpose To characterize the clinical profile, surgical outcomes, and prognostic factors of acute acquired V-pattern esotropia (AAVE), a rare, understudied subtype of strabismus, and compare its features to classic V-pattern esotropia (VE). Methods In this retrospective cohort study, 37 AAVE and 39 VE patients underwent standardized surgical correction. Outcomes included alignment, stereoacuity, and refractive status. Results Compared to VE, AAVE patients were significantly older at onset (8.5 vs. 3.0 years, p < .0001), exhibited milder hyperopia (+0.68D vs. +2.90D, p < .0001), and presented with larger preoperative esodeviations (44.1 vs. 36.8 PD, p = .0109). After 1-year follow-up, stereoacuity recovery was superior in AAVE patients (80.8% achieving good stereoacuity vs. 37.9% in VE; p = .0127), though surgical success rates did not differ significantly (73.0% vs. 64.1%, p = .4057). Conclusion AAVE represents a distinct diagnostic entity marked by acute onset, older age, and superior postoperative sensory recovery. Its unique refractive profile and association with modern near-work stressors raises consideration of modern visual stressors in pattern strabismus, supporting consideration of early surgical intervention to maximize binocular potential. These findings suggest that AAVE represents an important subtype in pediatric ophthalmology, supporting the need for updated clinical guidelines.

Evaluation of graded recession of inferior oblique muscle for correction of different grades of V-pattern strabismus

BackgroundV pattern identification is essential for proper strabismus management. Graded recession is a tailored approach to treat inferior oblique overaction (IOOA). The aim is to evaluate the efficacy of graded recession of inferior oblique muscle for correction of different grades of V pattern.MethodsForty patients from 3 to 18 years old with V pattern strabismus and primary IOOA were evaluated by prism cover test to assess the grade of IOOA and amplitude of V-pattern. Graded recession of IO muscle depends on the amplitude of the V-pattern and degree of IOOA. Eight mm recession for amplitude 15 PD to 20 PD and mild IOOA (10 PD-15 PD or + 1) ,10 mm recession for amplitude 20–30 PD and moderate IOOA (15–25 PD or + 2) and maximum recession for amplitude more than 30 PD and marked IOOA (≥ 25 PD or + 3). Simultaneous correction of the horizontal deviation was performed. Follow up after I week,1 month ,3 month and 6-month. Trial Registration Number (TRN) (NCT05786053) on 23/3/2023.ResultsThe mean age of the study patients was 9 ± 4.261. Twenty patients (50%) had V-pattern esotropia, 12 (30%) exotropia, 4 (10%) orthotropic and four (10%) had Dissociated vertical deviation (DVD). Four cases 10% were of grade 1, 20 cases (50%) grade 2 and 16 cases (40%) were of grade 3. Of eighty eyes, 66 eyes (82.5%) were fully corrected with no residual IOOA, and 14 eyes (17.5%) were under corrected. V-pattern was corrected in 28 cases 70% and only 12cases (30%) had residual V-pattern grade 1.ConclusionsGraded recession is an effective procedure for correction of V pattern strabismus with various grades of primary inferior oblique overaction. It can be tailored according to the the degree of IO overaction which is significantly related to the grade of V pattern. The 8 mm recession for IO was significantly related to recurrence or inadequate break of the V pattern in our studied cases. The grade of IOOA correlates with the amplitude of V-pattern. The amount of recession was planned according to preoperative IOOA and grade of V-pattern with frequent undercorrections obtained by the standard 8 mm recession. A + 2 overaction merits a 10-mm recession of the inferior oblique. A + 3 or + 4 overaction merits a 14-mm maximal recession.

Beneficial effects of preoperative intravitreal bevacizumab on trabeculectomy outcomes in neovascular glaucoma

This study aimed to investigate the effects of preoperative intravitreal bevacizumab (IVB) on outcomes in trabeculectomy for neovascular glaucoma (NVG). Charts for 52 NVG eyes of 52 consecutive patients who received primary trabeculectomy with mitomycin C (MMC) were reviewed. Postoperative follow-up periods for all patients were > or = 4 months. Thirty-two consecutive eyes were treated without IVB (control group) and 20 consecutive eyes received IVB (1.25 mg) 10 +/- 11 days before trabeculectomy (IVB group). The main outcome measures were postoperative intraocular pressure (IOP) and incidence of postoperative complications. Surgical success was defined as IOP< 21 mmHg with or without medication (qualified or complete success, respectively). Failure was defined as IOP exceeding these criteria, phthisis bulbi, loss of light perception or additional glaucoma surgeries. Kaplan-Meier survival analysis with the log-rank test was performed to compare surgical success rates between the two groups. Complete and qualified success rates at 6 months were 95% versus 50% and 95% versus 75% in the IVB and control groups, respectively. The IVB group achieved significantly better surgical success rates than the control group (complete success, p < 0.001; qualified success, p = 0.026). Postoperative hyphaema on day 1 or hyphaema with a duration of > 1 week occurred significantly less frequently in the IVB group than in the control group (p = 0.009, p = 0.014, respectively). The incidence of serious complications such as endophthalmitis, phthisis bulbi and a marked decrease in visual acuity did not increase in the IVB group. This retrospective study showed that preoperative IVB decreased postoperative hyphaema and increased surgical success rates, and thus may be an effective adjunct to trabeculectomy in NVG.

Long-Term Safety and Reliability of Using Tutoplast-Processed Fascia Lata to Refine the Nasal Dorsum in Primary and Revision Rhinoplasty.

Tutoplast-processed fascia lata (TPFL) is an allograft substance employed alongside cartilaginous materials to achieve optimal augmentation in rhinoplasty. Alternatively, it can be utilized to conceal and smooth irregularities of the nasal dorsum during the procedure. Despite its application, there is limited available data on the extended safety of TPFL. Consequently, our objective was to assess the enduring long-term safety and reliability of TPFL in both primary and revision rhinoplasty. A retrospective cohort study was conducted on 300 patients who underwent primary and revision rhinoplasty with TPFL grafts. Baseline characteristics, types of surgical techniques, and surgical outcomes were evaluated. The surgical success and satisfaction rates were assessed as primary outcomes. The Statistical Package for Social Sciences (SPSS), version 27 for Windows, was used to conduct the statistical analysis. TPFL was used in both primary and revision rhinoplasty, with septal cartilage being the most common conjunction graft material. The overall surgical success rate was 97.3%, with only 2.7% of patients experiencing complications. The satisfaction rate was 92.7%, with no significant difference between primary and revision cases. The highest satisfaction rate was found in patients who underwent the crushed cartilage in fascia technique (96.3%). TPFL in conjunction with cartilaginous materials; is a safe and reliable option for dorsal augmentation and camouflage material in primary and revision rhinoplasty, with low complication and high patient satisfaction rates. Prognostic Factors for Successful Surgical Outcomes in Trochlear Nerve Palsy: A Retrospective Study and Literature Review.

Trochlear nerve palsy is a common cause of double vision, particularly vertical diplopia. Surgery might be necessary if the condition does not improve independently. The success of the surgery can vary based on the method employed, and clear factors to predict its effectiveness are not evident. This study evaluates surgical techniques, success rates, and prognostic factors for trochlear nerve palsy at a tertiary hospital. A retrospective chart review was conducted on patients undergoing strabismus surgery for trochlear nerve palsy at Phramongkutklao Hospital between April 2012 and July 2024. Collected data included demographics, visual acuity, stereopsis, etiology, preoperative angles, surgical methods, and postoperative outcomes. A literature review regarding surgical success and prognostic factors was also conducted. Seventy-two cases were included, with 79.2% involving decompensated congenital trochlear nerve palsy. The overall surgical success rate was 76.39%, and inferior oblique myectomy was the most common and effective procedure (44.4% of cases). Based on multivariate logistic regression analysis, a preoperative hypertropia of ≤15 prism diopters was the significant factor for predicting successful outcomes in this study (OR 5.13, 95% CI 1.19-22.18). Inferior oblique muscle surgery effectively addresses small-angle deviations in trochlear nerve palsy. A <15 prism diopters vertical deviation strongly predicted positive surgical outcomes in this study. Further studies are needed to compare surgical techniques and explore additional prognostic factors to optimize long-term outcomes and improve patient care.

Early High Postoperative Intraocular Pressure Improved the Predictability of Trabeculectomy in Patients with Primary Open-Angle Glaucoma: A Retrospective Cohort Study

Introduction: The aim of this study was to investigate the effect of different levels of early intraocular pressure (IOP) on long-term outcomes of patients with primary open-angle glaucoma (POAG) who were treated with primary trabeculectomy. Methods: This was a retrospective cohort study, with a total of 74 patients (90 eyes) with POAG who were treated with trabeculectomy surgery at a single center from 2021 to 2022. Based on IOP at 1 day after surgery, they were divided into the high IOP group (≥15 mm Hg) and the low IOP group (<15 mm Hg). The Student’s t test and a chi-square test were used to compare the differences between the two groups. The generalized estimating equation model compared the included binocular data to adjust for internal correlations between the two eyes of the same subject. The Kaplan-Meier method compared the surgical success rates of the two groups. Besides, to identify the relationship between IOP at different postoperative time points and IOP at 1-year follow-up, the Spearman correlation analysis was established. Results: A total of 90 eyes (74 patients) with POAG were included in this investigation. After a 1-year follow-up, both the qualified surgical success rates in the high IOP group and the low IOP group were 92.3% and 92.2%, respectively, and there was no statistically significant difference between them (p = 0.890). The rate of bleb massage in the high IOP group was 82.1% (32 eyes) which was significantly higher than in the low IOP group (19.6%, 10 eyes) (p < 0.001). In the Spearman correlation analysis, it was observed that in the high IOP group, the significant correlation with IOP at 1 year emerged at 1 week after surgery (r = 0.496, p = 0.002) and a higher correlation at 1 month after surgery (r = 0.515, p = 0.002). In the low IOP group, the significant correlation with IOP at 1 year emerged at 3 month after surgery (r = 0.314, p = 0.038) and a higher correlation at 6 month after surgery (r = 0.592, p < 0.001). Conclusion: In the POAG patients with primary trabeculectomy, we demonstrated that compared with the low IOP group, the high IOP group could provide a similar surgical success rate while requiring a higher frequency of bleb massage. In addition, in the high IOP group, IOP at 1 month after surgery predicted long-term IOP control outcomes.

Baerveldt-350 implant versus Ahmed valve for refractory glaucoma: a case-controlled comparison.

To compare the efficacy of intraocular pressure (IOP) control of the Baerveldt-350 implant with tube ligature and the Ahmed valve in patients with refractory glaucoma. Four hundred seventy glaucoma drainage device procedures from July 1995 to July 2001 (6 years) were reviewed retrospectively. Thirty-two cases of Baerveldt-350 implantation performed in patients with glaucoma refractory to medical treatment and filtering procedures, without previous drainage device or cyclodestructive procedures, and with a minimum of 1-year follow-up were identified. Thirty-two cases of Ahmed valve implantation were matched case by case with 32 Baerveldt-350 cases for age, race, gender, glaucoma subtype, previous ocular history, preoperative IOP, and surgeon who performed the implantation. The two groups were compared for IOP control, visual outcome, complication rate, and surgical success rate. Surgical success rate was defined as an IOP reduction greater than or equal to 30% and final IOP more than 5 mm Hg and less than 22 mm Hg, without devastating complications. Over a follow-up period of 1 year, no statistically significant differences were detected between the Baerveldt-350 implant versus Ahmed valve for IOP control (12.1 +/- 5.3 mm Hg vs. 13.6 +/- 5.6 mm Hg respectively, at a power of 90% to detect a difference of 3.2 mm Hg between the two groups and P = 0.05), surgical success rate (65.6% vs. 65.6% respectively, complete and qualified combined), postoperative hypotony rate (37.5% vs. 34.4% respectively), and visual acuity changes of more than 1 line in Snellen visual acuity (43.3% vs. 29.0% respectively). In a case-controlled comparison, the Baerveldt-350 implant and the Ahmed valve had similar IOP control and surgical outcomes in patients with refractory glaucoma at 1-year follow-up.

Comparison of horizontal muscle transposition and Inferior oblique weakening combined with horizontal surgery in V-pattern exotropia.

The objective of this study was to compare the effects of horizontal muscle transposition and inferior oblique weakening combined with horizontal surgery performed for V-pattern exotropia (XT) and to determine the most successful approach. In our retrospective study, 52 patients who underwent horizontal surgery due to V-pattern XT and were followed up for at least 6 months were divided into two groups. Group 1 (n = 26) consisted of patients who underwent vertical transposition of the rectus muscles combined with horizontal surgery, and group 2 (n = 26) consisted of patients who underwent inferior oblique weakening combined with horizontal surgery. The two groups were compared before and after surgery in terms of visual acuity, refractive errors, deviation angles, pattern strabismus, fusion, stereopsis, over/under correction and surgical success. There was no difference between the two groups in terms of age, follow-up period, visual acuity, refractive errors, fusion or stereopsis. The amount of horizontal deviation was smaller in group 2 at the 1-6-month follow-ups. There was no difference between the two groups in terms of pattern collapse; however, the amount of pattern postoperatively was lower in group 2. The two groups were similar in terms of overcorrection; however, the undercorrection rate was higher in group 1. The surgical success rate was higher in group 2. Inferior oblique weakening combined with horizontal surgery due to V-pattern XT was found to be superior to vertical transposition of the horizontal muscles in terms of surgical success and the amount of pattern postoperatively.

Causal Brain Network Predicts Surgical Outcomes in Patients With Drug-Resistant Epilepsy: A Retrospective Comparative Study.

Network neuroscience, especially causal brain network, has facilitated drug-resistant epilepsy (DRE) studies, while surgical success rate in patients with DRE is still limited, varying from 30% ∼ 70 %. Predicting surgical outcomes can provide additional guidance to adjust treatment plans in time for poorly predicted curative effects. In this retrospective study, we aim to systematically explore biomarkers for surgical outcomes by causal brain network methods and multicenter datasets. Electrocorticogram (ECoG) recordings from 17 DRE patients with 58 seizures were included. Ictal ECoG within clinically annotated epileptogenic zone (EZ) and non-epileptogenic zone (NEZ) were separately computed using six different algorithms to construct causal brain networks. All the brain network results were divided into two groups, successful and failed surgeries. Statistical results based on the Mann-Whitney-U-test show that: causal connectivity of α -frequency band ( 8 ∼ 13 Hz) in EZ calculated by convergent cross mapping (CCM) gains the most significant differences between the surgical success and failure groups, with a P value of 7.85e-08 and Cohen's d effect size of 0.77. CCM-defined EZ brain network can also distinguish the successful and failed surgeries considering clinical covariates (clinical centers, DRE types) with [Formula: see text]. Based on the brain network features, machine learning models were developed to predict the surgical outcomes. Among them, the SVM classifier with Gaussian kernel function and Bayesian optimization demonstrates the highest average accuracy of 84.48% by 5-fold cross-validation, further indicating that the CCM-defined EZ brain network is a reliable biomarker for predicting DRE surgical outcomes.

Long-term surgical outcome of median nerve injuries

Background: Median nerve integrity is a prerequisite of normal hand function. The median nerve is a frequently injured nerve, and recent evidence indicates that nerve surgery may produce acceptable outcomes. The present study provides a framework of long-term surgical outcomes of median nerve injuries and specifies independent predictors of motor and sensory recovery. Materials and Methods: In the retrospective study, patients with median nerve injuries undergoing the nerve surgery were reviewed. Mechanism of injury, level of injury (arm, elbow/forearm, and wrist), type of injury and lesion, type of surgical repair, and the time interval from injury to surgery were assessed. The long-term follow-up of motor recovery, sensory recovery, and quality of life was done. Results: A total of 106 cases with median nerve injuries undergoing nerve surgery were included in this study. Most injuries were at wrist level with a higher frequency of sharp and not in continuity lesions. There was a significant association between the three outcomes (motor recovery, sensory recovery, and quality of life) and smoking, addiction, type of injury, type of lesion, and type of nerve repair. There was a significant correlation between quality of life and muscle force recovery and between quality of life and sensory recovery. Muscle force recovery was significantly correlated with sensory recovery. Conclusions: The findings of the present study suggest that male and young subjects, lesions in continuity, and injuries treated without grafting may show better surgical outcomes.

Influence of the Phenylephrine Test in Outcomes of External Levator Advancement for Involutional Ptosis.

This study aims to evaluate the influence of preoperative phenylephrine testing on the surgical outcome of patients undergoing surgery for involutional ptosis by external levator advancement. This was an observational, monocentric, retrospective study. Fifty-one eyelids from 32 patients, who had surgery between January 2018 and May 2023, were included for analysis. Preoperative clinical examination data were collected. Evaluation was performed at 1 month postoperatively. Surgical success was defined by a postoperative margin reflex distance between 3 and 5 mm inclusive. Symmetry success was defined by a difference in margin reflex distance between the 2 upper eyelids of no more than 1 mm. The surgical success rate was 86%. A positive preoperative phenylephrine test was significantly associated with a better surgical success rate ( p = 0.01), including on symmetry ( p = 0.01). The secondary outcomes, namely preoperative margin reflex distance, function of the upper eyelid levator muscle, and unilaterality of surgery, were not statistically associated with surgical outcome. The phenylephrine test is a predictive factor of surgical success in patients undergoing external levator advancement. Our study suggests that patients with a negative phenylephrine test should be overcorrected intraoperatively.

Three-year results of XEN gel stent implantation in the treatment of glaucoma

Objective: To evaluate the long-term efficacy and safety of XEN gel stent implantation in patients with refractory glaucoma. Methods: A retrospective case series study was conducted. Clinical data were collected from patients who received medical treatment at the First Affiliated Hospital of Fujian Medical University between January 2020 and September 2020 and underwent XEN gel stent implantation by the same surgeon. Preoperative and postoperative clinical parameters, including the intraocular pressure (IOP), number of ocular hypotensive medications, best-corrected visual acuity, surgical success rates, status of filtering blebs, and complications, were recorded at 1 day, 1 week, 1, 2, 3, 6, 12, 18, 24, and 36 months after surgery. Statistical analyses were performed using the analysis of variance (ANOVA), the least significant difference (LSD) method, and the Fisher's precision probability test. Results: A total of 8 patients (8 eyes) with glaucoma meeting the criteria were included, consisting of 6 males (6 eyes) and 2 females (2 eyes). The preoperative IOP reached (30.88±12.17) mmHg (1 mmHg=0.133 kPa), and the IOP at all postoperative follow-up time points was significantly lower than the preoperative value (P<0.05). At 3 years after surgery, the IOP was (14.38±1.60) mmHg, with a reduction of 53.43% [(16.50±12.07) mmHg]. Only 2 patients required monotherapy for IOP control, and there was no significant decline in best-corrected visual acuity in 7 patients. The surgical outcome at 3 years postoperatively was a complete success in 6 patients, a qualified success in 1 patient, and a failure in 1 patient. The filtering blebs were generally flat and diffuse, with only mild to moderate vascularization. One patient presented elevated IOP at 3 months, and after a subsequent revision of the XEN gel stent, the IOP became stable with the administration of travoprost eye drops. Conclusion: The XEN gel stent implantation proved to effectively reduce the IOP in glaucoma patients, with a high degree of safety demonstrated up to 3-year long-term follow-up.

Outcomes and Factors Associated with Successful Strabismus Surgery for Abducens Nerve Palsies: A Retrospective Study and Literature Review.

Several studies reported surgical outcomes for abducens nerve palsy, but information on factors that affect treatment success remains lacking. These factors are crucial for developing a treatment plan and providing disease counseling. This study aimed to investigate the outcomes of strabismus surgery for abducens nerve palsy and determine the factors that influence its success, including a review of relevant literature. This retrospective analysis included abducens nerve palsy cases, focusing on surgical interventions and relevant patient data, at the outpatient clinics of Phramongkutklao Hospital from April 1, 2012, to April 30, 2022. A relevant literature review included the surgical success rate and factors that influence surgical outcomes. This study enrolled 32 patients, including 19 with partial and 13 with complete abducens nerve palsy. The overall success rate of strabismus surgery was 78.1%. Trauma was the leading cause of abducens nerve palsy in this population (28.13%). Fisher's exact and Mann-Whitney U-tests revealed that shorter abducens nerve palsy onset and smaller preoperative angle were significantly associated with successful surgical outcomes of strabismus surgery among the groups. In contrast, subgroup analysis revealed that only preoperative smaller angles were significantly associated with good surgical outcomes in horizontal strabismus surgery. However, the vertical rectus muscle transposition group demonstrated no significant factors. The literature review revealed that the success rate of surgery in abducens nerve palsy was 25%-82.6% for horizontal rectus muscle surgery and 46.2%-91% for rectus muscle transposition. The surgical success rate for abducens nerve palsy reached 78.1%, including 78.95% for partial and 76.92% for complete abducens nerve palsy.
