Abstract
Objective To investigate the clinical diagnosis and treatment of primary gallbladder carcinoma presenting as acute abdomen. Methods Clinical feature, pathological data, surgical methods and treatment results of 23 cases primary gallbladder carcinoma presenting as acute abdomen were analysised retrospectively. Results Eighteen cases of 23 cases primary gallbladder carcinoma presenting as acute abdomen were presented as acute;2 cases presented as pyloric obstruction; 2 cases presented as gallbladder perforation; 1 cases presented as peritoneal hemorrhage. Preoperative diagnosis: only 5 cases were diagnosised primary gallbladder carcinoma; other cases were wrongly diagnosised acute abdomen. 23 cases were treated acute operation. surgical methods: 4 cases were radical cholecystectomy; 3 cases were radical cholecystectomy plus common bile duct drainage; 12 cases received palliative operation, 2 cases were only underwent biopsy due to intraperitoneal metastasis. 18 cases were followed-up from 1 year to 3 years after operation,1 year survival rate were 33.4 %, 3 year survival rate were10.2%. Conclusion In order to avoid misdiagnosis,the risk factors of gallbladder carcinoma should be emphasized. During operation of primary gallbladder carcinoma presenting as acute abdomen,the routine examination of resected gallbladder and suspected lesions of frozen slicing are necessary. After diagnosis of gallbladder carcinoma were established, radical resection should be perform for better prognosis. Key words: Acute abdomen; Primary gallbladder carcinoma; Diagnosis
