To investigate factors influencing maternal health care utilisation in western rural China and its relation to income before (2002) and after (2007) introducing a new rural health insurance system (NCMS). Data from cross-sectional household-based health surveys carried out in ten western rural provinces of China in 2003 and 2008 were used in the study. The study population comprised women giving birth in 2002 or 2007, with 917 and 809 births, respectively. Correlations between outcomes and explanatory variables were studied by logistic regression models and a log-linear model. Between 2002 and 2007, having no any pre-natal visit decreased from 25% to 12% (difference 13%, 95% CI 10-17%); facility-based delivery increased from 45% to 80% (difference 35%, 95% CI 29-37%); and differences in using pre-natal and delivery care between the income groups narrowed. In a logistic regression analysis, women with lower education, from minority groups, or high parity were less likely to use pre-natal and delivery care in 2007. The expenditure for facility-based delivery increased over the period, but the out-of-pocket expenditure for delivery as a percentage of the annual household income decreased. In 2007, it was 14% in the low-income group. NCMS participation was found positively correlated with lower out-of-pocket expenditure for facility-based delivery (coefficient -1.14 P < 0.05) in 2007. Facility-based delivery greatly increased between 2002 and 2007, coinciding with the introduction of the NCMS. The rural poor were still facing substantial payment for facility-based delivery, although NCMS participation reduced the out-of-pocket expenditure on average.

A bibliometric analysis of current research, hotspots, and development trends was used to develop an overall framework of mechanisms of alpine grassland degradation on the Qinghai-Tibet Plateau. This investigation includes data from 1,330 articles on alpine grassland degradation on the Qinghai-Tibet Plateau, acquired from the Chinese Science Citation Database (CSCD) and Web of Science Core Collection (WOS). Research was divided into three themes: spatial scope and management of typical grassland degradation problems, dynamic mechanisms of grassland degradation and effects of ecological engineering, and grassland degradation risk based on remote sensing technology. The results of the analysis showed that the research can be summarized into three aspects: typical grassland degradation identification, dynamic mechanism analysis of grassland degradation, and grassland ecosystem stability strategy. The main findings can summarized, as follows: (1) Ecological analyses using the river source as a typical region defined the formation of "black soil beach" type degraded grasslands in the region, and promoted the ecological environment management and protection of the alpine grassland by discussing the causes of regional ecological environment changes; (2) Dynamic mechanism analyses of climate change and characteristics analyses of grassland vegetation-soil degradation revealed that alpine grassland degradation is the result of multiple main factors; and (3) Risk prediction methods for grassland degradation, methods of grassland management and sustainable countermeasures for agriculture and animal husbandry development, and the development of a comprehensive index of influencing factors on grassland degradation all indicate that selecting the right grassland restoration measures is the key to grassland restoration. Remote sensing monitoring and high-throughput sequencing technology should be used in future research on grassland ecosystems. In addition, multiscale, multidimensional, and multidisciplinary systematic research methods and long-term series data mining could help identify the characteristics and causes of alpine grassland system degradation. These findings can help identify a more effective coordination of landscape, water, lake, field, forest, grass, and sand management for the prevention of alpine grassland degradation.

This study aims to investigate the factors contributing to Molar Incisor Hypomineralization (MIH), with a focus on maternal health during pregnancy, delivery complications, and postnatal health and nutritional factors in children. A cross-sectional study was conducted involving detailed surveys completed by mothers regarding their health during pregnancy, delivery experiences, and their children's early health and nutritional history. Clinical examinations of the children were performed to diagnose MIH according to established criteria. Statistical analyses were conducted using IBM SPSS 29.0, including descriptive statistics and correlation analyses to identify associations between MIH and various maternal and child health factors. The study found that 36.7% of mothers experienced illnesses during pregnancy, with gestational diabetes and hypertension being the most common conditions. Complications during delivery were observed in 3.3% of cases, and 16.7% of mothers had premature births. Common health issues in children included respiratory infections (26.7%), fever (23.3%), and otitis media (16.7%). Notably, dental defects were most prevalent in children with kidney disorders (66.7%), digestive system disorders (45.8%), and severe diarrhea (43.8%). Significant correlations were identified between MIH and these maternal and child health factors, highlighting the importance of comprehensive prenatal and postnatal care. The study underscores the multifactorial nature of MIH, revealing significant associations with maternal illnesses, delivery complications, and early childhood health issues. These findings emphasize the need for targeted preventive measures and early interventions to manage MIH effectively, particularly in children at higher risk due to compromised maternal or child health.

This study aimed to determine whether a history of depressive disorders is associated with use and costs of prenatal care among pregnant women in Taiwan. Participants were mothers with singleton births between 2004 and 2006 (N=23,290), some of whom (N=614) had received care for depression in the year before conception but not during pregnancy. The mean number of prenatal care visits was 8.50 and associated costs were $NT 51,187 for pregnant women with a history of depressive disorders and 9.17 visits and $NT 27,998, respectively, for those without such a history. After adjustment for age, monthly income, medical conditions, and obstetric complications, mothers with a history of depression were significantly less likely to receive prenatal care (relative risk=.94, 95% confidence interval=.92-.97, p<.001). However, women with a history of depression had $NT 22,494 higher prenatal care costs than mothers without a history of depression. Pregnant women with a history of depressive disorders had fewer prenatal care visits but higher prenatal care costs. Physicians should consider screening to identify pregnant women with a history of depressive disorders.

As part of an evaluation of an in-hospital birth centre, 576 expectant fathers were randomly allocated to birth centre care and 567 to standard maternity care in the earlier part of their partner's pregnancy. A questionnaire mailed 2 months after the birth asked about satisfaction with care, experience of labour and birth and adaptation to fatherhood. The response rates were 99% and 94% in the birth centre group and standard care group, respectively. The birth centre group had more positive experiences of intrapartum and postpartum care than the standard care group. Attitudes by staff were assessed as more positive, the fathers felt they were treated with greater respect, and during labour the midwife was more supportive of their needs as partners. However, birth centre care did not seem to have any major effect on the fathers' experience of labour and birth, or any long-term effect on adaptation during the first 2 months. Measures of physical and mental well-being, experience of fatherhood, involvement in infant care and utilization of paternal leave did not differ statistically between the trial groups.
