Abstract

The attendee will leam about the relative value of radiologic techniques in the evaluation of ovarian and adnexal masses with attention to the distinguishing features of hydrosalpinx, endometriosis, functional cysts and neoplasms. The principles for evaluating ovarian masses with a pattern recognition approach will be discussed as weil as the specific imaging features of sorne ovarian masses. In the adnexae, radiologists can confidently diagnose functional ovarian cysts, endometriomas, tubal disorders and peritoneal inclusion cysts. With few exceptions such as fibromas and teratomas, radiologic features of most ovarian neoplasms not diagnostic and pathologic evaluation is the goal standard. However, use of a pattern recognition approach may narrow the differential diagnosis and triage the patient to the appropriate surgeon. When faced with an adnexal mass, the list of potential diagnoses is long. Sonography is the first line imaging test performed. MR and CT are useful for problem solving and staging of malignancy. The most common ovarian and adnexal lesions in women of reproductive age are benign and non neoplastic. It is important to be aware of the imaging appearances of these benign lesions in order to guide appropriate management and suggest further imaging when this will add value. There are many subtypes of ovarian neoplasms but a component of pattern recognition may allow a more concise differential diagnosis.
