Abstract Introduction: The annual Brazilian Dialysis Survey (BDS) plays an important role in informing and shaping national health policies. Objective: To present the 2024 epidemiological findings from the BDS conducted by the Brazilian Society of Nephrology (BSN) and compare them with previous years. Methods: A survey was conducted among Brazilian chronic dialysis centers through voluntary participation, utilizing an online questionnaire to assess clinical and epidemiological characteristics of dialysis patients, as well as dialysis center attributes. For specific estimates of prevalence, incidence, and funding source, a nationally representative random sample of dialysis centers stratified by geographic region was selected (n = 258). Results: A total of 386 dialysis centers (42.7%) voluntarily responded to the online questionnaire, and 162 centers from the randomly selected centers provided data. On July 1st, 2024, the estimated number of dialysis patients was 172,585, with 52,944 new patients starting dialysis in 2024. The estimated prevalence and incidence rates per million population (pmp) were 812 and 249, respectively. Among prevalent patients, 87.3% were undergoing hemodialysis, 7.1% hemodiafiltration, and 5.6% peritoneal dialysis. Compared to the previous year, there was an increase in catheter use for hemodialysis vascular access, along with higher prevalence rates of anemia, hyperphosphatemia, hyperkalemia, and low Kt/V. The estimated crude annual mortality rate was 16.5%. Conclusions: Data from a random sample of dialysis centers indicate a continued rise in the number and prevalence of chronic dialysis patients in Brazil. Worsening trends in permanent vascular access, dialysis adequacy, and metabolic control underscore the need for targeted improvements in patient care.

The Brazilian Dialysis Survey (BDS) is an important source of national data about people on chronic dialysis that contributes to the formulation of health policies regarding kidney failure. To report the 2021 epidemiological data from the BDS of the Brazilian Society of Nephrology (BSN). A survey was carried out in Brazilian chronic dialysis centers using an online questionnaire covering clinical and epidemiological aspects of patients in chronic dialysis, data on dialysis therapy, characteristics of dialysis centers, and the impact of the COVID-19 pandemic of 2021. Thirty percent (n = 252) of the centers answered the questionnaire. In July 2021, the estimated total number of patients on dialysis was 148,363. The estimated prevalence and incidence rates of patients per million population (pmp) were 696 and 224, respectively. Of the prevalent patients, 94.2% were on hemodialysis (HD) (1.8% of these on hemodiafiltration), and 5.8% on peritoneal dialysis (PD); 21% were on the transplant waiting list. The incidence rate of confirmed COVID-19 between January and July 2021 was 1,236/10,000 dialysis patients, and the case-fatality rate reached 25.5%. Up to July 2021, 88.6% of patients had received at least one dose of the anti-SARS-CoV-2 vaccine. The estimated overall and COVID-19 crude annual mortality rates were 22.3% and 5.3%, respectively. The absolute number and the prevalence rate of patients on chronic dialysis continue to increase. Most dialysis patients were vaccinated against COVID-19 during the year. The COVID-19 pandemic was associated to the overall mortality rate.

Challenges of vascular access in a new dialysis centre - Uyo experience

Introduction of dialysis has prolonged the lives of end-stage renal disease patients. To maintain these patients on long term dialysis, permanent vascular access procedures capable of allowing flow of >200ml of blood/minute, are required. Without permanent vascular access, patients are subjected to repeated attempts for cannulation to provide temporary vascular access during every session of haemodialysis, risked with numerous vascular access related complications. The objective of the study was to analyse the problems of vascular access in our new dialysis centre and plan intervention. Case notes and dialysis records of consecutive patients who underwent haemodialysis in our dialysis centre during its first one year were used to collect data into proforma, and these were analysed. There were 60 patients who underwent a total of 254 sessions of haemodialysis during the period. Their ages ranged from 12-72 years. There were 38 males and 22 females. There were 57 patients with end-stage renal disease and three with acute renal failure. Only 5% of the patients underwent dialysis through a permanent vascular access representing 8% of dialysis. The remaining 95% of patients undergoing 92% of haemodialysis utilised temporary vascular accesses. Complications arising from vascular access were noted in 24.0% of dialysis and these included failed or difficult cannulation, poor flow, haematoma, haemorrhage, kinked catheter, thrombosis and infection. The ratio of temporary to permanent vascular access of 92:8 noted in our dialysis centre was unacceptably high compared to the internationally recommended 15:85.

The Brazilian Dialysis Survey (BDS) is an annual national survey about patients on chronic dialysis that contributes to health policies. To report the 2022 epidemiological data from the BDS of the Brazilian Society of Nephrology (BSN). A survey was carried out in Brazilian chronic dialysis centers using an online questionnaire that included clinical and epidemiological aspects of patients on chronic dialysis, dialysis therapy data, and dialysis center characteristics. Overall, 28% (n = 243) of the centers answered the questionnaire. In July 2022, the estimated total number of patients on dialysis was 153,831. The estimated prevalence and incidence rates of patients per million population (pmp) were 758 and 214, respectively. Of the prevalent patients, 95.3% were on hemodialysis (HD, 4.6% of these on hemodiafiltration) and 4.7% on peritoneal dialysis (PD). Only 1.3% of patients were not vaccinated against COVID-19. The prevalence of anemia (Hb < 10g/dL) was 27% and hyperphosphatemia (P > 5.5mg/dL) reached 30%. The estimated overall crude annual mortality rate was 17.1%. The absolute number and prevalence rate of patients on chronic dialysis continue to increase. A growing number of patients were receiving hemodiafiltration. The mortality rate decreased, probably due to the end of COVID-19 pandemic. The conclusions were drawn in the context of relatively low voluntary participation, which imposed methodological limitations on our estimates.

Of the prevalent patients, 88.2% were on hemodialysis, 8.0% on hemodialfiltration, and 3.8% on peritoneal dialysis. The prevalence of anemia (Hb &lt; 10g/dL) was 29% and hyperphosphatemia (P &gt; 5.5mg/dL), 30%. There was an increase in the frequency of use of cinacalcet and paricalcitol. The estimated overall crude annual mortality rate was 16.2%. Conclusions: Estimates from a random sample of dialysis facilities show that the absolute number and the prevalence rate of patients on chronic dialysis continue to increase. A growing number of patients were undergoing hemodiafiltration and using cinacalcet and paricalcitol for hyperparathyroidism treatment.
