Abstract

A large segment of the primarily older individuals in skilled nursing facilities (SNF) have cardiovascular disease and experience multiple hospital admissions to acute care hospitals. Many of these individuals may require and benefit from cardiovascular expertise, yet the need has not been well characterized. We hypothesized that older individuals from SNFs would have a greater burden of cardiovascular disease than those admitted from home.
