Abstract
Building a successful medication therapy management practice requires pharmacists to communicate drug therapy problems to prescribers along with appropriate clinical recommendations to resolve the problems. Communication with other health care providers should be more than just transactional (i.e., in response to a problem). It should be relational so that a collaborative working relationship (CWR) is developed.
