Abstract

A 70-year-old man presented in September, 1990, with a 2-week history of a dry cough, haemoptysis, breathlessness, myalgia, and flitting arthralgia. On examination he was not feverish but was cyanosed and tachypnoeic. He had a systolic murmur consistent with aortic sclerosis and crackles over the mid and lower zones of both lungs. There were no joint abnormalities, muscle tenderness, or cutaneous manifestations of vasculitis. Arterial blood gases were: pO2 6·5 kPa and pCO2 4·9 kPa breathing air at rest.
