Abstract

Adequate hospital care for older people with acute medical disorders requires assessment to detect those patients at highest risk of functional decline. Specialist multidisciplinary care and early planning for discharge and follow up are vital to prevent loss of independence and maintain quality of life. Delaying functional loss and increasing the chances of living at home are at least as important as reducing fatality in frail older people.
