Answer: Actinomyces radicidentis bacteremia. The Gram stain of the isolate from the subculture of the positive blood culture on Trypticase soy agar plates supplemented with 5% sheep blood (5% SBA) at 48 h of incubation at 35°C in CO2 showed organisms consistent with Gram-positive cocci in clusters
