Advances in the application of registration methods of computer-aided dynamic intraoperative navigation in oral and maxillofacial surgery: A systematic review.

With the rapid development of medical imaging technology, computer-assisted dynamic intraoperative navigation (CADIN) technology has been introduced into the field of oral and maxillofacial surgery due to its technological features of accurately localizing key anatomical structures during surgery. Registration is a key step in CADIN technology. In different application scenarios, the choice of the registration method directly determines the accuracy of the navigation feedback, which in turn affects the effectiveness and safety of the entire surgery. In this paper, by searching and analyzing the database of articles on the application of CADIN technology in the field of oral and maxillofacial surgery for the years 2019-2025. The inclusion criteria are the application, optimization and system design of CADIN technology in oral and maxillofacial surgery. After screening 1069 articles, 42 articles were finally included. An analysis of the articles included in the study revealed that trauma and facial reconstruction guided by CADIN technology are hot research topics in the field of CADIN technology in oral and maxillofacial surgery. There are few reports on the use of CADIN technology to guide the endodontic treatment. In addition, the largest number of studies performed the registration process using the markerless. A review of the literature reveals that CADIN technology has great potential for practical clinical application in the field of oral and maxillofacial surgery and that the selection of appropriate registration methods can improve the accuracy of oral and maxillofacial surgical procedures.

Oral and maxillofacial surgery has seen various advancements throughout years. From dental implants to 3D printing, every advancement this field has witnessed has taken it one step ahead into the future. Reconstructive surgeries in oral and maxillofacial surgery have evolved drastically over the years. Till now, autografts are considered the best possible alternative to soft tissue or hard tissue defects. The advent of 3D bioprinting has not only broadened the vision of surgeons, physicians, and bioengineers but also enkindled the hope for a better future in reconstructive surgeries. All in all, the facial structures are full of complexities and loss of tissues in maxillofacial area is much common. 3D bioprinting with the ability to print a tissue with pre-defined arrangement of cells having its own vascularization can prove to be a valuable tool in the field of maxillofacial surgeries. Although still it has a long way to go to make it accessible for all, 3D bioprinting can be considered an asset for the field of oral and maxillofacial surgery. Top 100 cited systematic reviews and meta-analyses in the major journals of oral and maxillofacial surgery: a bibliometric analysis.

The aim of this bibliometric research was to identify and analyze the top 100 cited systematic reviews in the field of oral and maxillofacial surgery in order to guide any professional level with interest in this topic and to map the current trends the field of oral and maxillofacial surgery. Using the Web of Science database without restrictions on publication year or language, a bibliometric analysis was performed for the five major journals of oral and maxillofacial surgery: International Journal of Oral and Maxillofacial Surgery (IJOMS), Journal of Oral and Maxillofacial Surgery (JOMS), Journal of Cranio-maxillofacial Surgery (JCMS),British Journal of Oral & Maxillofacial Surgery (BJOMS), and Oral Surgery, Oral Medicine, Oral Pathology and Oral Radiology (Triple-O). The most top-cited systematic review was published in 2015 with a total of 200 citations on survival and success rates of dental implants, consistent with the finding that "pre- and peri-implant surgery and dental implantology," and "craniomaxillofacial deformities and cosmetic surgery" were the most frequently cited topics (22% each). The majority of top cited papers were published in IJOMS (43%), followed by JOMS (34%), Triple-O (8%), JCMS(8%) and BJOMS(7%). The highest number of contributions was from the Netherlands, followed by Italy and USA. The outcome of this article can be used as a source of information and to guide not just researchers but also clinicians and students to which areas are trending in the field of oral and maxillofacial surgery, thus also having a large impact on the field of oral and maxillofacial surgery. However, this article cannot reflect the quality of the included systematic reviews.

Oral and Maxillofacial Surgery: Why the double degree?

The International Association of oral and maxillofacial Surgeons (IAOMS) is committed to quality patient care. The education of oral and maxillofacial surgeons has, however, been in constant evolution ever since the inception of this speciality; and this state of flux reflects the dynamic expansion of its scope. Oral surgery has a unique relationship with medicine and dentistry as it straddles both professions. To understand this dynamic expansion, it is pertinent to review the present scope of oral and maxillofacial surgery. Today an oral and maxillofacial surgeon’s training includes management of trauma, surgical reconstruction of acquired and developmental deformities, temporomandibular joint surgery, dentoalveolar surgery, preprosthetic surgery including implants, management of odontogenic infections, management of oral pathology, and administration of general anaesthesia and sedation. The oral and maxillofacial surgeon routinely treats patients with systemic diseases such as acute and chronic alcoholism, diabetes mellitus, hypertension, cardiovascular and pulmonary disease, neurological problems, as well as drug abusers. The new requirement for oral and maxillofacial surgery training programmes is 48 months with rotations in anaesthesia, medicine and surgery, and a minimum of 30 months in the oral and maxillofacial surgery service. Eighteen months of off-service rotations, most or all of which are taken at an intern or resident level instead of medical student or clerk level, give oral and maxillofacial surgery (OMFS) residents high quality medical education. Their rotation in general surgery and surgical sub-specialties result in a broad exposure to the concepts and principles of surgery beyond that provided by the oral and maxillofacial surgery rotation. It is the dental education that differentiates oral and maxillofacial surgeons from other surgical specialties; therefore, formal dental education culminating in a D.D.S or D.M.D. degree is strongly encouraged. Background: Oral and Maxillofacial Surgery department is a diverse field in dentistry. Record maintenance has been established as one of the key factors in the success and integrity of health care institutes.Objective: The objective of the study was to evaluate the records of oral and maxillofacial surgery cases reported to oral and maxillofacial surgery department, Abbasi Shaheed Hospital and oral surgery OPD of Karachi Medical and Dental College.Methods: Cross sectional study was conducted in at ASH and KMDC from July 2019 to September 2019. The data from January 2017 to July 2019 was retrospectively noted through electronic surgical record of ASH and records of the Oral Surgery OPD of KMDC. Inclusion criteria was patients records of both genders of 5–70 years age, having complaint of any oral or dental pathology or pathologies, trauma and impactions. Data was calculated manually by calculating frequencies and percentages for the trauma, impaction and pathology cases of patients.Results: In 2017, 239 cases were treated under general anesthesia from which trauma 11. 45% (n=11), followed by 48. 11% (n=115) cases of oral pathologies, total 11.7% (n=28) cases of complicated exodontias. In 2018, among 211, 51.1% (n=108) cases were trauma followed by 39.3% (n=83) cases of oral pathologies, whereas, total 9.4% (n=20) complicated exodontias cases were observed. During 2019 (January to July), 168 cases 36.2% (n=62) cases were diagnosed as trauma, in oral pathology, overall 36.2% (n=62) cases were surgically excised. Total 23.2% (n=39) complicated exodontias. In 2017, 25122 cases were reported in Surgery OPD of Karachi Medical and Dental College. Total 36.2% (n=9097) teeth were extracted from which 1.93% (n=486) cases were surgical impaction. On the other hand, 1.65% (n=416) patients were treated through minor oral surgeries. In 2018, 29008 cases were reported in Surgery OPD. Total 42.7% (n=12377) teeth were extracted from which 0.92% (n=268) cases were surgical impaction. On the other hand, 0.71% (n=208) patients were treated through minor surgeries. In 2019, January till July 13028 cases were reported in Surgery OPD. Total 42.6% (n=5559) teeth were extracted from which 0.66% (n=87) cases were surgical impaction. On the other hand, 0.68% (n=89) patients were treated through minor surgeries.Conclusion: It has been concluded that evaluation of the records of oral and maxillofacial surgery cases reported to oral and maxillofacial surgery department, Abbasi Shaheed Hospital and oral surgery OPD of Karachi Medical and Dental College were high and appropriate measures should be taken in order to manage these problems timely and effectively.

Mesenchymal Stem Cells in Oral and Maxillofacial Surgery: A Systematic Review of Clinical Applications and Regenerative Outcomes.

Aim: This systematic review aims to evaluate the use of mesenchymal stem cells, particularly those derived from bone marrow, adipose tissue, and dental pulp in maxillofacial and oral surgery, focusing on their regenerative potential, clinical applications, and integration with biomaterials. Introduction: Mesenchymal stem cells are multipotent stem cells known for their immunomodulatory and regenerative abilities. Their low immunogenicity and differentiation capacity make them ideal for treating craniofacial defects and enhancing soft tissue repair. Materials and Methods: The review followed PRISMA guidelines and was registered in PROSPERO. The literature was searched across PubMed, Scopus, and Web of Science from 2009 to 2024. Twelve studies met the inclusion criteria and were analyzed for clinical efficacy and methodological quality. Results: Clinical trials demonstrated the safety and regenerative benefits of mesenchymal stem cell in bone and soft tissue reconstruction. Adipose-derived stem cell and dental pulp stem cell showed favorable outcomes in angiogenesis and healing, while bone marrow's cell proved effective in bone regeneration, particularly when combined with scaffolds. Discussion and Conclusions: Although results are promising, limitations remain in consistency and long-term outcomes. Optimizing scaffold integration, preservation methods, and delivery techniques is crucial. Mesenchymal stem cell-based therapies represent a powerful, minimally invasive alternative to traditional grafting in oral and maxillofacial surgery.

Current Management of Patients Taking Immunosuppressive Medication and Targeted Therapies: National Survey Within Oral Surgery and Oral and Maxillofacial Surgery

ABSTRACTIntroductionSystemic immunosuppressive medication and targeted therapies (IMTT) can increase the risk of surgical procedures to patients. The risks of continuing their systemic IMTT medication during the pre‐, peri‐ and post‐operative period must be weighed up against the risk of stopping their medication. Currently, there is a paucity of evidence and national guidance on the management of oral surgery and oral and maxillofacial surgery patients taking IMTT.Materials and MethodsA cross‐sectional online survey was distributed to clinicians within primary and secondary care who were providing oral surgery or oral and maxillofacial surgery services. Distribution was through the British Association of Oral Surgery (BAOS) and the British Association of Oral and Maxillofacial Surgery (BAOMS). The survey explored current practice and available guidance with regard to alteration of IMTT dosing intervals and contact with the prescribing clinician.ResultsOne hundred and forty‐six surveys were completed by clinicians working in oral surgery or maxillofacial surgery settings, and general dental practitioners with a special interest in oral surgery. The majority of responses were collected from clinicians practising in the UK both in primary and secondary care. The results show that clinicians frequently treat patients taking IMTT. Currently, there are a variety of approaches to managing patients on IMTT and a lack of standardisation across the specialities.ConclusionFurther research and evidence‐based national guidance, specifically for oral surgery and oral and maxillofacial surgery, would provide clarity on the optimal care for those taking IMTT.
