Abstract

Objectives: Stroke is the leading cause of death in China. However, no detailed and systematic assessment has been made for stroke burden and secular trend. In this study, we initiate a comprehensive assessment to illustrate secular change of stroke burden in China. Methods: Data about the incidence, mortality and disability-adjusted life years (DALYs) of stroke from 1990 to 2016 was obtained from IHME-GHDx, which also categorized in gender, subtypes and age groups. The secular trend of stroke burden was analyzed by subtypes, and illustrated secular change of stroke risk population by age and sex with population pyramid. Change of stroke burden during that period was compared among China, the U.S. and the world. Results: In 2016, the age-standardization rate (ASR) for incidence, mortality and QALYs rate of stroke in China is 403.08, 130.94, and 2825.37, respectively. The secular tendency of overall ASR of incidence remained stable, however, the ASR for incidence of ischemic stroke increased, and that of hemorrhagic stroke decreased. Besides, the secular tendency of ASR for mortality and DALYs rate decreased, which was mainly accounted by the decreasing of hemorrhagic stroke. In both males and females, the incidence rate of ischemic and hemorrhagic stroke increased in 15–49 and 50–59 age groups, while mortality decreased in all age groups. From 1990 to 2016, the ASR for incidence of stroke increased in China but decreased worldwide and in the U.S., and the decrease of ASR of mortality in China was more noticeable than that of the U.S. and the world. Conclusion: In China, the ASR of incidence of ischemic stroke increased and that of hemorrhagic stroke decreased. A higher priority of stroke prevention should focus on reducing incidence especially for ischemic stroke and young or middle-aged population.
