Abstract

It is clear from the literature that a community-based approach to rehabilitation following traumatic brain injury is both essential and cost-effective. A model is presented for community-based rehabilitation which is client needs-led and interprofessional. The model described uses a client-led needs assessment as a basis for a contract identifying immediate, short- and long-term goals, and an intervention programme to achieve these in clients' homes and supported by their families. Interprofessional records are kept and regular re-assessment leads to goal and programme modification if necessary. Case studies are presented to show how activities of living scores can be dramatically increased when the model and programme of stimulation are implemented and systematically evaluated. Community-based rehabilitation for people recovering from traumatic brain injury requires commitment and dedication by all partners, and it will be shown that high levels of functioning can be restored using this approach.
