Abstract
虫垂原発の良性腺腫はまれな疾患であり，術前に診断可能であった報告は少ない．今回われわれは，大腸内視鏡検査にて術前診断をしたうえで，腹腔鏡補助下に切除した虫垂管状腺腫の1例を経験したので，若干の文献的考察を加え報告する．症例は54歳の男性．便潜血反応陽性のため当院内科にて大腸内視鏡検査を施行したところ，虫垂開口部に径8mm大の隆起性病変を認めた．生検時腫瘍を牽引すると有茎性であり，根部は虫垂内であった．生検の結果管状腺腫と診断されたため，当科紹介となり手術を施行した．腹腔鏡補助下に虫垂を臍部ポート創に持ち上げ，遺残が生じないように虫垂付着部近傍の盲腸を含めて虫垂を切除した．切除標本上虫垂根部に8×4×6mm大の有茎性腫瘍を認め，病理組織学的検査にて軽度の異型を伴う虫垂管状腺腫と診断された．虫垂原発の良性腺腫に対し腹腔鏡下手術を施行した症例の報告は少ないが，自験例は腹腔鏡下手術の良い適応と考えられた．
