Abstract
症例は53歳，男性．右大腿部の疼痛があり当院整形外科を受診．CT検査で，6cmに腫大した甲状腺左葉とともに第2胸椎・右大腿骨の溶骨性変化を認めた．同日外科紹介となり甲状腺超音波検査で，左葉腫瘤より穿刺細胞診を行ったが良性所見であった．右大腿骨の病的骨折をきたしたため手術を行ったが，その際の骨生検で甲状腺癌骨転移の診断を得た．第2胸椎と右大腿骨近位部に外照射を行った後に，甲状腺全摘術+D1を行った．病理組織所見では，6cmに腫大した甲状腺左葉内のほとんどは腺腫様甲状腺腫所見であったが，一部に微小な濾胞型乳頭癌が認められた．左甲状腺微小濾胞型乳頭癌，T1a，N0，M1（第2胸椎，右大腿骨），Ex0，Stage IV Cと最終診断した．術後，ゾレドロン酸4mgの投与，100mCiの放射性ヨード内用療法を2回行った．術後1年の現在，右大腿骨転移部，第2胸椎転移部の増悪はなく，ADL・QOLの低下をきたしていない．
