Abstract

We arthroscopically studied the chondral lesions of the patellofemoral joint in 37 cases who had undergone 47 patellar dislocations and in 16 cases who had 21 episodes of subluxation. We defined the chondral lesions as Grade 2 and 3 using the grading system proposed by Noyes and Stabler. Chondral lesions were seen in 29 patellae (61.7%) and 24 femurs (51.1%) of the patellar dislocation group, and in 11 patellae (52.4%) and 4 femurs (19.0%) of the patellar subluxation group. In patients with patellar dislocation, the lesion was more often located medially and centrally. In comparison in patients with the patellar subluxation, the lesion was more often located on the lateral part of the patella. The frequency and grade of the chondral lesion increased with time after onset. We consider that realignment of the patellofemoral joint in the early stage of patellar dislocation and subluxation is necessary for preventing degenerative change of the patellofemoral joint.
