Abstract
最近の医療情勢の変化から, day surgeryに対応できる手術が俄かに注目され, 術者の手技の熟達度とは関係なく, 頻繁に実施されるようになってきた.day surgeryの増加に伴って, 患者側からの不満も増えている.したがって, このような状況が肛門病学の本質に適った変化であるか否かを真剣に検討する必要があると考える.筆者らは長年, 外来手術だけで肛門疾患に対処してきたが, 主として古典的な非観血療法を実施してきた.技術的には, この特殊性がday surgeryを可能にさせてきたのかもしれないが, 最も大切にしていることは患者と担当医が相互に直接連絡できる体勢をとっていることである.患者らが安心して自宅で入院と同じ状態で治療できるシステムの形成が最重要と考えている.今後, 本気でday surgeryを実行しようとする術者は, 術後の一定期間は24時間拘束される覚悟がなければ, 安易に実践するべきではないと警鐘を鳴らしたい.
