The article presents the results of epizootological and epidemiological monitoring of dirofilariasis in the south of Russia in the period 2000-2024. During this period, 4378 blood samples of dogs living and serving in the south of Russia were examined. In general, the prevalence of dirofilaria in definitive hosts decreased from 21.0 ± 1.1% (2000-2008) to 13.6 ± 0.9% (2017-2024). Long-term observations have shown that the level of infection of domestic dogs directly depends on the conditions of their keeping. Over the past twenty years, the prevalence of dirofilariasis in service and apartment dogs has decreased by 4 times, and a stray animal continue to grow and currently reaches 25.0%. Based on the analysis of 317 medical records of patients with dirofilariasis treated in the clinic of infectious and parasitic diseases for the period 2000-2024, a decrease in the number of patients with dirofilariasis on average per year from 19.56±0.28 (2000-2008) to 6.38±1.50 (2017-2024) people was noted. Long-term observations of dirofilariasis in animals and humans indicate a positive epidemiological and epizootological situation in the south of Russia. Official statistics indicate a stable incidence rate of the population in Russia of 0.04-0.09 per 100 thousand people, which is due to the expansion of the distribution area of dirofilaria to the northern and north-eastern regions of the country and the urbanization of this invasion.

The prevalence of canine dirofilariasis in China: a systematic review and meta-analysis

BackgroundDirofilariasis, the disease caused by Dirofilaria spp., and in particular by Dirofilaria immitis and Dirofilaria repens in canines, occurs frequently in canids and felids, and occasionally in humans, in temperate, sub-tropical and tropical regions globally. Although highly effective, safe and convenient preventive medicines have been available for the treatment of dirofilariasis for the past three decades, the disease remains a major veterinary and public health concern in endemic areas. The insect vectors, host-parasite relationships and interactions of Dirofilaria spp. have received little attention in China, and there is very little information in English regarding the prevalence of dirofilariasis in animals and humans in the country. The aim of this systematic review and meta-analysis is to evaluate the status of canine dirofilariasis in China based on the available literature in English and in Chinese.MethodsWe systematically searched five databases for epidemiologic studies on the prevalence of canine dirofilariasis in China and finally selected 42 studies eligible for inclusion in the systematic review and meta-analysis. The meta-analysis was performed using the random effects model in the meta package in R v4.2.1.ResultsThe random effects model gave a pooled and weighted prevalence of Dirofilaria infection among dogs in China in the past 100 years of 13.8% (2896/51,313, 95% confidence interval 8.2–20.4%) with a high level of heterogeneity (I2 = 99.5%).ConclusionsOur analyses indicated that the prevalence of canine dirofilariasis in China has gradually declined, but that the range of Dirofilaria spp. has expanded. Older and outdoor dogs presented a higher rate of positive infection. The findings indicated that more attention should be paid to host factors for the effective control and management of this disease.Graphical

Tick-borne infections in the south of Russia: modern epidemiological situation, new approach to create “forecasting” and “explaining” morbidity models (in astrakhan rickettsiosis fever and crimean-congo hemorrhagic fever)

The article presents a description of the current tick-borne infection epidemiological situation in the south of Russia from the years 2013 to 2022, proposes a new approach to develop forecasting models for morbidity dynamics of Astrakhan rickettsial fever (ARF) and Crimean hemorrhagic fever (СCHF) in the Astrakhan region and presents data assessing 2022 explaining models for the Stavropol Territory and Astrakhan Region. Materials and methods. A comprehensive research was performed using epidemiological analysis and non-parametric statistical methods. The data assessing tick-borne infections epidemic process manifestations were retrieved from ARF and CCHF morbidity databases (developed as a project) and documents of infectious disease focus epidemiological examination provided by the departments of Rospotrebnadzor in the subjects of the Southern and North Caucasian Federal Districts. Morbidity models were developed using the Bayes theorem and Walds sequential statistical analysis, with a preliminary calculation of indicators informativeness by the Kullback method. The values of climatic factors from the database of the Center for Collective Use IKI-monitoring of the Space Research Institute of the Russian Academy of Sciences were used. Results. The results of the study indicate persistence of serious epidemiological situation regarding rickettsiosis of the tick-borne spotted fever group, Q fever, tick-borne borreliosis and CCHF in the south of Russia. Almost all tick-borne infections nosological forms in children under 14 years (including young children and infants) were widely involved in the epidemic process, which belong to patients at risk for a complicated disease course due to complicated diagnostics and treatment. The annual registration of tick-borne infections cases in the resort areas, with the subsequent occurrence of imported cases in other, including non-endemic regions poses a serious problem. The proposed forecasting models allow to predict the CСHF and ARF morbidity for each administrative district of the Astrakhan region with up to 91.7% accuracy. The explaining models CСHF accuracy for the Stavropol Territory and Astrakhan Region, when tested in 2022, was 88.5 and 83.3%, respectively, for ARF 91.7%. Conclusions. The further continuation of forecasting and explaining models verification for planning preventive measures and propose similar steps for tick-borne borreliosis and Q fever to epidemiological tick-borne infections to stabilize situation in the south of Russia.

The current situation of echinococcosis in the Far East and South of Russia

Objective: to assess the epidemic situation of echinococcosis in the period 2012–2022 in the endemic territories of the Far Eastern Federal District and South of Russia.Materials and methods: based on the data of the annual statistical reporting forms No. 2 "Information on infectious and parasitic diseases", data from the Reference Center for Monitoring Larval Helminthiasis of Rostov Research Institute Microbiology and Parasitology of Rospotrebnadzor, an analysis of the incidence of echinococcosis in the territories of the Far East and South of Russia in the period 2012–2022 was carried out. For seroepidemiological examination of a conditionally healthy population by the ELISA method, the diagnostic kit "Echinococcus-IgG-ELISA-BEST" (JSC "Vector-Best", Novosibirsk) was used.Results: it is shown that the epidemiological situation of echinococcosis is characterized by pronounced territorial unevenness. In the south of Russia, epidemiological problems with echinococcosis persisted in Kalmykia (0.74–2.19 cases per 100,000 population) and in the Astrakhan region (0.60–1.48 cases per 100,000 population). Among the relatively healthy population, the average seropositivity to E.granulosus antigens in the Far East was 5.8%, in the south of Russia — 2,6%. Conclusions: the data obtained indicate an unfavorable epidemiological situation for echinococcosis in a number of regions of the Far East and South of Russia.

DISEASE OF SERVICE DOGS WITH DIROFILARIASIS ON THE TERRITORY OF THE RUSSIAN FEDERATION

Service dogs are an important source of distribution and maintenance of dirofilariasis foci in various territories due to the frequent change of their places of service and movement throughout Russia. The aim of our work was to study the dynamics of infection of working dogs and their role in the spread of dirofilariasis. On the basis of Federal Budget Institution of Science of Rostov Institute of Microbiology and Parasitology, studies of the invasion of dogs by dirofilariasis have been conducted from 2000 to the present. In 2018, 159 blood samples of dogs from different territories of the European part of Russia were examined using the blood larvoscopy method after concentration in 3% acetic acid with subsequent staining according to Romanovsky-Giemsa. The results of a survey of dogs that arrived in Rostov-on-Don to protect socially significant objects in June 2018 showed that the prevalence of infection of working dogs was 13%. Animals from the territories of central Russia were infected in 16.6% of cases, the same indicator in dogs from the territories of southern Russia was 9.1%. The infection of service dogs of Dirofilaria repens was 8.7%, Dirofilaria immitis – 1.4%, and mixed infection was detected in 2.9% of the dogs. In the south of Russia, the prevalence of dogs with Dirofilaria immitis and mixed invasion is prevalent, while Dirofilaria repens dominates in the central zone. Analyzing the data obtained and previous research results, one can note the fact that active preventive measures have led to a decrease in the incidence of dirofilariasis among working dogs. However, the average extensiveness of this dog invasion remains high.

INFECTION OF PEOPLE OF THE RUSSIAN FEDERATION WITH DIROFILARIASIS

Wide circulation of the causative agent of dirofilariasis in the natural environment and the lack of appropriate measures to identify and deworm infected animals – obligate definitive hosts (domestic dogs and cats) promotes infestation by the human helminth infections. The aim of the study is to analyze the incidence of dirofilariasis in people according to official statistics. The analysis of 52 surveillance maps collected for 2019, 2020 and official statistics on the incidence of parasitic diseases for 2019 was carried out. The analysis of the surveillance maps showed that dirofilariasis is most often registered in the south of Russia (the share of the Southern and Volga Federal Districts accounts for more than 50% of registered patients), the main group of patients with dirofilariasis falls on 30–39 year old people and older 60 years old or more (23.1%, respectively). The structure of patients with dirofilariasis is dominated by women (65.2%). Infestation is more often registered in urban residents (75%). Almost half of the removed dirofilaria, were localized in the head area (weight share of 44.2%). Among the removed helminths, females predominated (80.8%), of which 72.7% were immature. Cases of human infestation with dirofilariasis are registered throughout the Russian Federation, with the exception of the northern regions. Despite the increase in the proportion of Dirofilaria immitis, especially in the south of Russia, the disease caused by Dirofilaria repens and extremely rarely D. immitis are registered in people on the territory of the Russian Federation. It is possible that in this case there is a hypodiagnostic invasion of D. immitis.

P-1804. Evaluating Implementation Outcomes of Stewardship Interventions for Rapid Blood Culture Diagnostics

Background Rapid diagnostic testing for bloodstream infections, when coupled with antimicrobial stewardship (AS) intervention, has demonstrated improved clinical outcomes. Yet, data regarding implementation outcomes and the necessary strategies for these interventions are limited.Table 1:Verigene Gram Positive Blood Culture Nucleic Acid Test (BC-GP) results Pre-intervention and During the Intervention Frequency and percentage of BC-GP results during the pre-intervention phase and intervention phases are reported. Coagulase negative staphylococci includes Staphylococcus species, Staphylococcus epidermidis, methicillin susceptible Staphylococcus epidermidis, and methicillin resistant Staphylococcus epidermidis BC-GP results. Methods A pre-post quasi-experimental design was used to compare children with positive blood culture results. Pre-intervention causative organisms were identified using the Verigene Gram Positive Blood Culture Nucleic Acid Test (BC-GP) and reported to clinicians by a microbiology technologist. Post-intervention organisms were identified and reported similarly, in addition to 24 hours a day/7 days a week (24/7) AS intervention. AS members received BC-GP notifications and communicated antibiotic recommendations to the primary team. Hospital stakeholders were identified in the pre-intervention phase to assess divisional needs. The primary outcome was time to targeted therapy (TTT), calculated as time from BC-GP result to order of predetermined narrowest antibiotic regimen or discontinuation of therapy. TTT was compared by time-series analyses. Secondary outcomes included acceptability, appropriateness, and feasibility, assessed using validated surveys completed by pediatric infectious diseases (ID) clinicians and non-ID clinicians who received the AS intervention. Proportion of Patients Receiving Targeted Antibiotic Therapy Over Time A Kaplan-Meier analysis with a hazard function for time to targeted therapy (TTT) in children with positive blood cultures. Cases in the intervention phase (11/2023-04/2024) are represented in blue, and preintervention (10/2022-10/2023) controls are represented in orange. Censored cases included patients who did not receive targeted antibiotic therapy within their hospital admission. Log rank test demonstrated a statistically significant reduction in TTT in the intervention group versus the pre-intervention group, with a larger proportion of patients in the intervention group receiving targeted antibiotic therapy more rapidly than patients in the pre-intervention group (P-value of 0.013). Results The median TTT decreased from 15.9 hours before intervention to 0.9 hours with AS intervention (P=0.013). Validated surveys were completed by 23 ID and 38 non-ID clinicians. Over 80% of ID and non-ID clinicians agreed or strongly agreed that the intervention was acceptable and appropriate. However, significant differences were noted in all feasibility domains, including “doable,” “easy to apply,” “implementable,” and “possible" measures. Specifically, 39% of ID clinicians reported the intervention was “easy” or “very easy” to apply, as compared to 84% of non-ID clinicians (P < 0.0001). Conclusion 24/7 AS intervention paired with rapid blood culture diagnostics is associated with reduced TTT and is deemed appropriate and acceptable by ID and non-ID clinicians. The difference in feasibility perception by ID and non-ID clinicians highlights implementation challenges, and warrants evaluation of future strategies. Frequency and percentage of responses from infectious diseases (ID) clinicians in comparison to non-ID clinicians for the three domains of successful implementation outcomes including 1) acceptability, 2) appropriateness, and 3) feasibility. Agreement on the acceptability and appropriateness for the AS intervention was observed among ID and non-ID clinicians with no significant difference between the two groups. Statistically significant differences were noted among responses between ID and non-ID clinicians in the feasibility domain. Epidemiological Risk of Dirofilariasis in the Voronezh Region

Background. Dirofilariasis is a vector-borne helminthiasis that tends to expand spatial boundaries in areas with temperate and cold climates. In the Voronezh oblast, located in the Central Black Earth Region of the Russian Federation there were previously isolated reports of cases of dirofilariasis in humans and dogs, but this issue was not specifically studied.Aims. To determine the degree of infection of dogs with Dirofilaria immitis and Dirofilaria repens in the Voronezh oblast in order to determine the level of epidemiological risk.Materials and methods. For seven years, targeted work was carried out to study the spread of different types of dirofilariae in dogs in the Voronezh region. Blood from 3 498 dogs of different breeds, ages, conditions of keeping and household use was examined. Dirofilaria larvae were counted using the Fuchs-Rosenthal camera. The type of dirofilaria larvae isolated from the blood of dogs was determined by the histochemical method. The antigen of the imaginal form of D. immitis was detected using a chromatographic test system the ImmunoRun Antigen Detection Kit Caninae Heartworm Biogal chromatographic test system (Israel). Statistical processing of the material was carried out in the Microsoft Excel 2010 program. Studies were carried out with the consent of animal owners and in accordance with the European Convention for the Protection of Vertebral Animals used for experimental and other scientific purposes (Strasbourg, 1986).Results. As a result of the conducted studies, it was found that in the Voronezh oblast, the average infection of domestic dogs (Canis familiaris) with dirofilariasis is 14.14 ± 0.31 %. The indicator depends on the conditions of keeping animals and makes 27.15 % in stray dogs, 25.26 % – in service dogs and 10.54 % – in apartment dogs. In service dogs, the species D. repens occurs in 52.39 % of cases, D. immitis – in 42.95 %, mixtinvasia – in 4.80 % of cases. In stray dogs, the species D. repens prevails (80.10 %). The second type of D. immitis was established in 10.0 % of cases. At the same time, both types were also established in 10.0 % of cases. The maximum number of cases of dirofilariasis occurs in urban dogs (65.87 %), which is due to the high density of their population and close contact with the vector of transmission in urbanized areas. The seasonal dynamics of infestation is determined by the year-round detection of cases with a peak in October (46.77 %). The number of cases of dirofilariasis increases with age, in dogs from one to six years old it reaches maximum of 63.26 %. There is an epidemiological risk of dirofilariasis in the Voronezh oblast. This is due to the existence and active functioning of synanthropic and natural foci of invasion on the territory of the Voronezh oblast.

Differences between Infectious Diseases--Certified Physicians and General Medicine--Certified Physicians in the Level of Comfort with Providing Primary Care to Patients

Human immunodeficiency virus (HIV)-related mortality has decreased because of highly active antiretroviral therapy. As the life expectancy of HIV-infected patients has increased, the management of comorbid disease in such patients has become a more important concern. We examined the level of comfort self-reported by experts in HIV medicine with prescribing medications to HIV-infected patients for hyperlipidemia, diabetes, hypertension, and depression. As part of a larger project (the Veterans Aging Cohort Study), physicians at infectious diseases (ID) clinics and physicians at general medical (GM) clinics were asked to complete a survey requesting information about demographic characteristics, training and certification received, and self-reported comfort with prescribing medications for patients with hyperlipidemia, diabetes, hypertension, and/or depression. Comfort was rated using a 5-point Likert scale, with scores of 4-5 classified as "comfortable." Of 150 attending physicians surveyed, 51 (34%) were ID certified, 33 (22%) were GM certified but practicing at an ID clinic, and 66 were GM certified and practicing at a GM clinic. Comorbid conditions were common among HIV-infected patients treated at the ID clinics (22% of these patients had hyperlipidemia, 17% had diabetes, 40% had hypertension, and 27% had depression). However, comfort with treating these conditions was less among physicians at the ID clinic. For example, comfort treating patients with hyperlipidemia was greater for GM-certified physicians at GM clinics than for GM-certified physicians and ID-certified physicians at ID clinics (98% vs. 73% and 71%, respectively; P < .0001 for trend). A similar pattern was seen for treating patients with diabetes and hypertension (P < .0001). Comfort with treating patients with depression was generally lower, particularly among physicians at ID clinics (P < .0001). We found that ID-certified physicians and GM-certified physicians at ID clinics reported less comfort prescribing medications for common comorbid conditions, compared with generalist physicians at GM clinics, despite a substantial prevalence of these conditions at the ID clinics. Methods are needed to increase physicians' level of comfort for prescribing treatment and/or to facilitate referral to other physicians for treatment.

P-611. Improving Rabies vaccination delivery and access: A Quality Improvement Project

Background Rabies remains a global public health concern.Barriers affecting rabies post exposure prophylaxis include lack of access to the rabies vaccine, coordination of care, and costs. Due to the challenges of vaccine access and coordination, most rabies vaccines are given in Emergency Centers (ECs). Barriers faced by patients in the EC include increased wait times, costs, and lack of appropriate referral or follow up coordination. We initiated a quality improvement project (QIP) to improve access to rabies vaccination and delivery of appropriate doses of the vaccine in the Infectious Diseases (ID) Clinic at our institution. Rabies post exposure prophylaxis in infectious diseases clinic Methods Our outcome measure was to increase the total number of patient encounters for rabies vaccination in the ID Clinic across the 3 campuses at TCH by 25% in 3 months. Process measures were 1) Increase the number of referrals placed to ID by EC on first encounter by 25% in 3 months. 2) Increase the percentage of patients seen in ID clinic for all subsequent doses. Balance measure was collecting surveys to evaluate patient satisfaction with the vaccine process, including any perceived barriers. Our PDSA cycles included: 1) Education on the process to the EC providers at each campus, pharmacy, and nursing leads 2) Creating an Epic smart phrase to include the rabies vaccine schedule with appropriate dates, ID clinic phone numbers, and rabies vaccine education for families. 3) Providing small cards with the Epic smart phrase and ID referral reminder to attach to EC provider workstations at each campus. 4) Request for a standardized order set to be built in Epic to include orders for HRIG (as indicated), rabies vaccine, and referral to ID. Results Figure 1 represents the percentage of vaccine encounters seen in the ID clinic with interventions marked. We had excellent patient satisfaction scores in the ID clinic related to less wait times, easy access and costs. There was a significant cost differential between the EC and ID clinic. Conclusion Rabies Vaccine encounters increased from 16% to 61% between September 2023 through April 2024 across all three campuses of the hospital. Vaccines were provided with appropriate indications. Patient satisfaction was high with significant cost savings. There are currently ongoing PDSA cycles to improve access to rabies vaccine further. The purpose of the research is to study the contamination of environmental objects and Toxocara canis transmission in the occurrence of the people’s risk of becoming infected. Materials and methods. 481 fecal samples of anonymous dogs were studied for the period from 2011 to 2015 in order to determine the rate of Toxocara sp. infection in dogs in various territories on the south of Russia (the Rostov Region, the Krasnodar Territory, the Republic of Adygea and Karachay-Cherkessia). Fecal samples were taken in the spring-summer time from the territories of private residences. Helminthovoscopic studies of fecal samples were carried out by the Fulleborn method using a saturated solution of sodium nitrate, and ether-formalin sedimentation method using concentrators of the MiniParasep system. To calculate the number of Toxocara sp. eggs in 1g of feces, a VIGIS camera was used. Seroepidemiological diagnostics of 5194 blood serums of southern Russia population deemed to be healthy was carried out in order to identify specific class G antibodies to T. canis for 2011–2018. Toxocara-IgG-IFA-BEST reagents were used as diagnostic test systems. Enzyme immunoassay was performed according to the instructions. Results and discussion. In the study of 481 samples of depersonalized feces of dogs in various territories of southern Russia, the proportion of positive samples averaged 29.31%, while the proportion of samples with T. canis eggs ranged from 6.0% in the Krasnodar Territory to 16.7% in the Republic of Adygea. Toxocara sp. eggs were found in 42.2% of samples. The analysis showed the fluctuations in the incidence of toxocarosis from 1.33 in 2014 to 2.19 in 2018 per 100 thousand. The seropositivity level of the examined individuals was in the range of 19.5–40.9% in the Rostov Region, 17.0–25.0% in the Astrakhan region, 21.78–37.11% in the Krasnodar Territory, 22.5–47.0% in the Republic of Adygea, 34.0–42.27% in the Karachay-Cherkess Republic, 13.8% in the Chechen Republic and 19.9% in the Republic of Crimea.

Knowing that dirofilariosis is an emerging zoonosis, little known and studied in Romania, the aim of the study was to determine the prevalence of Dirofilaria spp. infection in dogs, in the western part of Romania. Since there is no epidemiological study on Dirofilaria spp. infection in dogs in Romania by molecular techniques, we consider opportune to examine the positive samples by this technique. Between February 2008 and August 2009 a number of 188 blood samples, from dogs in Timis and Hunedoara Counties, were collected. From the total amount of samples 96 were from Timis County and 92 were collected from Deva dog shelter, Hunedoara County. Blood samples were examination by different methods: Fresh blood smear, modified Knott's method, Diro-test Speed® Diro/Heartworm (Bio Veto Test, France), ELISA technique (D. immitis antigen) (EVL Netherlands). The positive blood samples with microfilariae were examined by PCR technique used for specific detection of D. immitis and D. repens described by (Casiraghi et al., 2006), at the Faculty of Veterinary Medicine “Szent Istvan”, at Parasitology and Zoology Department, from Budapest, Hungary. All 8 dogs tested were positive for Dirofilaria repens infection

Prevalence of dirofilariasis in shepherd and stray dogs in Iranshahr, southeast of Iran

Dirofilariasis is a zoonotic parasitic disease, which its cause Dirofilaria immitis, a nematode transmitted by insects and a worldwide dissemination. Dogs and cats are the main hosts of this parasite. Therefore, this study was conducted to determine the prevalence of D. immitis in shepherd and stray dogs in Iranshahr city, southeast of Iran. In this study 49 shepherd dogs and 50 stray dogs selected randomly and the blood samples were taken and sent to parasitology laboratory. Thin and thick thin blood samples were prepared and stained with Giemsa method and modified knott method. The obtained data were analyzed by SPSS 18 statistical software. The overall prevalence of parasite was 30.3%, seven of 49 shepherd dogs (14/3%) and twenty three of 50 stray dogs (46%) were positive. From 30 positive samples, 14.3% of shepherd dogs and 50% of stray dogs were male, and 44.7% of stray dogs were female. 14% of infected shepherd dogs and 38.5% of infected stray dogs were indigenous, and 16.7% of infected shepherd dogs and 72.7% of infected stray dogs were hybrids. There was no statistically significant relationship between the prevalence of D. immitis with age, gender, breed and usage of anti-parasitic drugs. Due to the relatively high contamination of dogs in the city of Iranshahr with this parasite, veterinarians and public health professionals should be considered necessary and preventive measures in relation to this disease.

EPIZOOTOLOGIC MONITORING OF NATURAL-FOCAL INFECTIONS IN THE SOUTH OF RUSSIA IN 2015

Analyze epizootologic manifestations of natural-focal infections (NFI) in the south of Russia in 2015. Reports provided by administrations of Federal Service of Surveillance for Protection of Consumers Rights and Human Welfare, Centers of Hygiene and Epidemiology, Research Institutes for Plague Control and Station for Plague Control were used. Data processing was carried out using Excel program. A total of 37-738 samples of field material were studied, most of those - for tularemia (8268), Crimea hemorrhagic fever (6894) and West Nile fever (4875). Causative agents of 15 nosologic forms of NFI were detected during epizootologic examination of the territory of south of Russia, predominating amount of positive samples was obtained during study of material for tick-borne borreliosis (612), Crimea hemorrhagic fever (279) and tularemia (237). Analysis of epizootologic situation on natural- focal infections in the south of Russia gives evidence on the increase of activity of natural foci of bacterial (tularemia, tick-borne borreliosis), rickettsia (tick-borne spotted fevers) and viral infec- tions (Crimea hemorrhagic fever, West Nile fever, tick-borne viral encephalitis), and thus problems of prophylaxis of natural-focal infectious diseases and constant monitoring of natural foci in su- bjects of Southern, North-Caucasian and Crimea federal districts are of particular importance.

Objective. To analyze the incidence of infectious diseases (IDs) and healthcare resources spent on treatment of ID patients in the Republic of Tatarstan (RT) in 2005–2017. Materials and methods. The analysis of ID incidence in the RT was based on the official statistics (report forms No 2 and No12) for 2005–2017. To assess the situation with medical care for ID patients, we used final reports for the ID service of the Ministry of Health of the RT for 2005–2017. Results. Between 2005 and 2017, the prevalence of IDs in the RT decreased by 10.8%; the incidence of IDs decreased by 17% during this period (primarily in adults (18.7%). In rural and urban areas, the incidence decreased by 29.2% and 16.6%, respectively. The number of outpatient ID departments was reduced by 28.8%; the number of hospital beds was reduced by 50.2%; the number of ID specialists was reduced by 27.1%. These reductions limited the availability of healthcare for ID patients. Conclusion. We observed a decrease in the ID incidence in the RT in 2005–2017, as well as a reduction of staff and funding in ID clinics, which reduced availability of specialized medical care to patients with ID. We found a significant correlation between the ID incidence and situation with medical care in specialized healthcare institutions for ID. Геоинформационный мониторинг демографических процессов в регионах юга Европейской России

The paper presents the results of the demographic processes analyses study in the South of European part of Russia in comparison with national outcomes. The research is based on a GIS monitoring concept and reveals the main characteristics of population size dynamic pattern with the influence of prevailing reproduction processes. The main sources of data are the office for national statistics and various regional statistics sources. ESRI ArcGIS Spatial Analyst software platform is used as a primary analytical framework. The study case covers the time frame from 1959 to 2018. Developing map models of the population’s natural reproduction brings out the depopulation speed and direction as a primary trend in Russia’s demographic development. At the beginning of the XXI century’s third decade, there is a significant reduction in the number of regions with positive natural population growth. The regions with decreasing in population have prevailed. The positive natural population growth preserves mainly in national territorial subdivisions with an unfinished demographic transition. The demographic picture in the south of European part of Russia looks relatively propitious. Negative demographic trends arise with a delay and have not reached the critical levels yet. The obvious polarization of demographic space in the south of Russia has educed — there are two areas with unequal trends of population growth: South-East area with a sustainable increase in the population and a migration outflow, the North-West area with a natural decline in the population and an erratic positive migration balance. Though the area with a sustainable increase in the population is reducing, showing positive trends in Krasnodar Krai and Republic of Adygea. The ethnic republic’s migration outflow is stable.
