Abstract

Purpose. To present a clinical case of cataract phacoemulsification in a patient with a thermokeratoplasty in anamnesis. Material and methods. Patient M., went to the clinic with complaints of decreased vision, “fog” in front of both eyes, especially in front of the right one. In the anamnesis 30 years ago, she had thermokeratoplasty for high degree hypermetropia (+8.00D) in both eyes. Due to the existing complaints and the presence of cataracts in both eyes, cataract phacoemulsification was performed with the implantation of toric IOLs in both eyes. Results. When evaluating the results in the longterm postoperative period (1 year after surgery), it was revealed that despite the difficulties that the surgeon faces when calculating intraocular lenses in patients after keratorefractive operations, we managed to achieve high visual acuity. Conclusions. The presented clinical case indicates that the measurement of the refractive power of the cornea using a modern scanning keratotopograph, the axial length of the eye using a noncontact biometer, the use of modern formulas for calculating the IOL, as well as the experience of an ophthalmic surgeon, allow achieving high functional results in the rehabilitation of patients with this pathology.
